SUMMARY DESCRIPTION:

The Gateway Transition Coordinator is responsible for the initial response to the referral, completion of the Return to Community Assessment (RTCA) and Options Counseling, obtaining Gateway to Community Living (GCL) Informed Consent, and initiating the Individual Transition Plan (ITP) under the Alabama Community Living (ACL) Medicaid Waiver Home and Community-based Services Program (HCBS) within the SARPC area and surrounding counties following specific requirements as set out in ACT Medicaid Waiver guidelines.

SUPERVISORY CONTROLS:

Receives general supervision from the Medicaid Waiver Coordinator. Supervisor sets the overall objectives and employee and supervisor, in consultation, develop the deadlines, projects, and work to be done; employee is responsible for planning and carrying out the assignment; and work is reviewed only from an overall standpoint.

RESPONSIBILITIES AND DUTIES:
1. Serve as the first line for individuals who seek to transition from an institutionalized care settingback into the community under the Alabama Community Transitions (ACT) Waiver / Gateway toCommunity Living (GCL).
2. Responsible for initial response to the referral, completion of the Return to CommunityAssessment (RCA), Options Counseling, and obtaining GCL Informed Consent.
3. Initiate the Individual Transition Plan (ITP) through a person-centered process, ensure the waiverPlan of Care is integrated into the ITP, and is responsible for overall tracking of the ITP throughcompletion of implementation.
4. Work closely with the participant, the nursing facility discharge planner, and the waiver casemanager to monitor and facilitate the overall implementation of the ITP and its projectedtimeframes.
5. Ensure the GCL Office is informed of projected move/transition dates in a timely manner andensure completion of a Quality of Life (QoL) Survey is completed by the local Ombudsman.
6. Market and perform outreach regarding the ACT Waiver and transition procedures to include onevisit monthly to every regional facility.
7. Ensure compliance with state and federal standards and regulations.
8. Attend conferences, seminars, and other continuing education programs as required.
9. Other duties as required.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
1. Knowledge of social work programs and processes preferred.
2. Knowledge of community resources and support network available to clientele served.

3.Excellent communication skills, both orally and written.

4.Excellent organizational and time management skills.

5.Ability to relate to the elderly and their unique problems. Working knowledge of Medicaid eligibility and Nursing Home requirements preferred.

6.Knowledge and ability to operate office equipment including telephone, copy machine, fax machine, computer, and calculator.

7.Knowledge and ability to do basic arithmetic.

8.Ability to read and interpret guidelines and regulations and apply them to the job.

9.Bachelor’s Degree from an accredited four-year college or university in Social Work, Behavioral Sciences, Psychology, Geriatric Studies, or a related field. At least one (1) year of case management experience.

10.A valid driver’s license and a good driving record.

WORK CONDITIONS:
The work requires some physical exertion such as long periods of standing; walking over rough, uneven, or rocky surfaces; recurring bending, crouching, stooping, stretching, reaching, or similar activities; recurring lifting of moderately heavy items such as furniture, home goods, etc. This position requires driving on a regular basis.
