Job Description
General Purpose of Job :
Provides medical social work services for Agency patients, their families, and caregivers. Conducts ongoing medical social work in-services and support for department staff as needed.
Essential Duties :
This job description is intended to cover the minimum essential duties assigned on a regular basis. Team members may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Provides medical social casework under physician’s plan of care to patients and families receiving other skilled services. Confers with primary case manager and other professionals involved in providing care.
- Evaluates patient in-home situations, and prepares patient histories to augment available patient and family data in determining appropriate level of services and resources.
- Assesses the patient and family situation, considering patient’s prognosis, medical needs, and family’s ability to provide adequate care and support. Facilitates referral to other appropriate community resources when increased needs are identified.
- Supervises and directs Medical Social Worker – Bachelors per Conditions of Participation.
- Maintains necessary clinical records for patient home visits and clinical conferences within mandated time frames.
- Maintains current knowledge of community resources. Accesses those resources as needed during and after the provision of other home care services.
- Maintains professional standards, compliance with State, Federal (CMS conditions of participation) and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the SHC expectations – standards of care guidelines.
Job Requirements
General Requirements
• Current state of Michigan certification/registration or license as a social worker • Valid driver's license and automobile insurance. • Active American Heart Association BLS Certification upon hire.
Work Experience
• Minimum of two years experience in a community health/acute care setting or home health agency • Recent experience in home care setting, preferred
Education
• Masters Degree in social work from an accredited school of social work.
Specialized Knowledge and Skills
Abilities: • Ability to enter/exit patient homes that may/may not be handicap accessible • Demonstrated excellent interpersonal/communication skills, verbal and written. • Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job. • Demonstrated knowledge of medical social work methods with particular skill in case management; strong assessment problem solving and organization skills, and ability to adapt knowledge and skill to in-home settings. • Demonstrated ability to work with stressful situations and adjust to changing situations. • Demonstrated interpersonal skills to effectively and appropriately communicate with patients, families, physicians, and interdisciplinary team.
