TaskForce Prevention and Community Services is a multi-service, grassroots organization committed to improving the sexual health and overall well-being of LGBTQ+ youth of Color in Chicago.

TaskForce provides safe and affirming spaces for community, HIV/STI screening and education, case management, and connections to medical, housing, and other social services. As TaskForce continues to grow its programs and services, our team works across disciplines to help community members access the resources and support they need to thrive.

Position Summary

The Ryan White Case Manager supports people living with HIV in accessing, engaging in, and remaining connected to medical care and supportive services. Through client-centered assessment, individualized care planning, service coordination, advocacy, and ongoing follow-up, the Case Manager works in partnership with clients to identify priorities, navigate barriers to care, and strengthen stability and self-sufficiency.

The Case Manager maintains an assigned caseload, completes required assessments and reassessments, develops and updates individualized care plans, coordinates referrals across medical and supportive service providers, and maintains timely and accurate client documentation and program data.

Essential Responsibilities

Client Assessment & Individualized Care Planning

- Build trusting, respectful relationships with assigned clients and create an environment in which clients can identify their needs, priorities, and goals.
- Conduct client assessments and required reassessments to identify strengths, needs, barriers, resources, and changes in circumstances.
- Develop, monitor, and update individualized care plans with clients based on their priorities, needs, progress, and changing circumstances.
- Assess clients’ stability and self-sufficiency related to engagement in HIV primary care and support appropriate transitions or discharge when applicable program criteria are met.

Care Coordination, Referrals & Advocacy

- Support clients in accessing and remaining engaged in HIV primary care and other needed health and supportive services.
- Connect clients with appropriate medical, behavioral health, substance use, housing, and other community-based resources based on their individual needs and goals.
- Coordinate care with medical providers, behavioral health professionals, substance use service providers, and other organizations involved in a client’s care.
- Monitor referrals, address barriers to accessing services, and advocate with clients, providers, and community partners to support successful connections to care and resources.

Client Engagement & Retention in Care

- Maintain regular and appropriate communication with assigned clients to monitor progress, identify emerging needs, and support continuity of care.
- Conduct outreach and case-finding activities as needed to reconnect clients with care and supportive services.
- Provide client education and support related to health promotion, engagement in care, and medication adherence within the scope of the position.
- Promote client agency and shared decision-making by supporting clients in identifying their own goals, priorities, and next steps.

Documentation, Data & Program Accountability

- Maintain accurate, complete, and timely case notes, service records, assessments, care plans, and other required client documentation.
- Enter and maintain client-level data and units of service in designated data systems within established program timelines.
- Collect and submit required information for program monitoring, evaluation, reporting, and quality-improvement activities.
- Maintain client confidentiality and follow applicable TaskForce and Ryan White program policies, privacy requirements, documentation standards, and service requirements.

Team & Multidisciplinary Collaboration

- Participate actively in weekly multidisciplinary case conferences and other relevant team meetings.
- Collaborate with colleagues and external service providers to support coordinated, client-centered care.
- Seek consultation and escalate complex client situations to the Program Director or other appropriate leadership when additional guidance or support is needed.
- Contribute to a collaborative team environment through proactive communication, information sharing, and constructive feedback.

Qualifications

Minimum Qualifications

- Bachelor’s degree in social work, psychology, public health, or a related field and at least two years of HIV-related experience involving direct client contact; or at least six years of case management experience within the HIV sector.
- Experience supporting clients through education, counseling, case management, care coordination, or related direct-service activities.
- Strong written and verbal communication skills.
- Ability to maintain accurate and timely client documentation while managing multiple client needs and follow-up responsibilities.
- Ability to use Microsoft Office and learn and effectively use required client-data and case-management systems.
- Ability to work the required schedule, including two evenings per week until approximately 10:00 p.m.
- Valid driver’s license, if driving is required to perform the essential functions of the position.

Preferred Qualifications

- Experience providing case management, care coordination, or navigation services to people living with HIV.
- Experience supporting clients with health promotion and medication adherence.
- Familiarity with HIV-related medical care and supportive-service systems.
- Experience working collaboratively with medical, behavioral health, substance use, housing, or other community-based service providers.
- Knowledge of Chicago-area health and social-service resources.
- Experience providing culturally responsive and affirming services to LGBTQ+ communities and communities of color.

Work Environment, Schedule & Physical Demands

- Full-time, 40-hour-per-week position.
- Regular evening availability required, including working until approximately 10:00 p.m. two evenings per week when TaskForce is open.
- Regular interaction with clients, colleagues, community partners, and service providers.
- Requires discretion in handling confidential information and flexibility in responding to changing client and program needs.
- Ability to perform the essential functions of the position, with or without reasonable accommodation, which may include prolonged periods of sitting and computer-based work and occasionally lifting or moving materials weighing up to 20 pounds.

Compensation and Benefits

- Salary Range: $50,000 -55,000 annually, based on relevant experience and education.
