Job Title: Medical Scribe Job Description
The Medical Scribe supports physicians by accurately documenting patient encounters in real time within the electronic medical record. This role ensures complete and precise transcription of patient histories, physical examinations, procedures, test results, diagnoses, and treatment plans, allowing physicians to focus more fully on direct patient care. The position requires strong attention to detail, excellent written communication, and a solid understanding of medical terminology and clinical workflows.
Responsibilities
- Accompany the physician during patient encounters to document information in real time.
- Transcribe the physician-dictated patient history, including chief complaint, history of present illness, past medical history, family history, and social history.
- Transcribe the physicians physical examination findings and any procedures performed during the visit.
- Transcribe results of laboratory tests and radiographic studies as communicated by the physician.
- Transcribe patient education provided by the physician, including explanations of risks and benefits for procedures performed.
- Transcribe pending orders for referrals, diagnostic tests, medications, and procedures as dictated by the physician.
- Transcribe all diagnoses, symptoms, follow-up instructions, and prescription information as directed by the physician.
- Prepare and edit the scribed documentation to ensure accuracy, completeness, and clarity before physician review.
- Submit the completed documentation for physician review, make any required corrections, and finalize the note for physician signature.
- Maintain confidentiality of patient information and adhere to all clinical documentation standards and policies.
Essential Skills
- At least 6 months of experience as a Medical Scribe.
- Proficiency with electronic medical record (EMR) systems.
- Strong computer skills and the ability to learn new applications quickly.
- Excellent written communication skills, including accurate spelling, punctuation, and grammar.
- Solid understanding of medical terminology, anatomy and physiology, diagnostic procedures, pharmacology, and treatment assessment sufficient to accurately transcribe physician encounters.
- Strong attention to detail and accuracy in clinical documentation.
- Ability to follow directions closely and work efficiently in a fast-paced clinical setting.
Additional Skills & Qualifications
- College education in a related area of study is preferred.
- Experience with CS Link/Epic electronic medical record systems is preferred.
- Familiarity with clinical workflows in outpatient or research-based healthcare settings.
- Ability to maintain professionalism and discretion when working directly with patients and clinical staff.
Work Environment
This is a day shift position with typical hours from 8:00 a.m. to 4:00 p.m. The role is fully on-site in Los Angeles within a clinical research setting. The Medical Scribe works closely with physicians and other clinical staff in exam rooms and clinical work areas, using electronic medical record systems and computer-based tools throughout the day. The environment is professional, patient-focused, and fast-paced, requiring consistent attention to detail and adherence to clinical documentation standards. Dress and appearance should align with a professional clinical setting.

This is a Contract position based out of Marina del Rey, CA.
Pay and Benefits
The pay range for this position is $25.00 - $25.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
