About this role
The Palliative Care Intake RN supports the Comprehensive Care team by triaging incoming referrals and assigning to appropriate clinical disciplines and providers. This position also performs triage from patient or caregiver calls and coordinates timely care with the PCP or Comprehensive Care team provider.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Review the medical record upon referral to care coordination to ensure timely, cost-effective services at the appropriate level of care, including palliative and hospice services.
Perform complete assessment of patient's current health status, including barriers to achieving optimal health, and available resources. Identify potential gaps in care based on the assessment.
Collaborate with care teams, patients, and caregivers to achieve plan of care outcomes and improve patient outcomes.
Communication with referring providers to clarify referral needs
Evaluate barriers to attainment of self-management goals and develop strategies to overcome.
Document in the medical record as indicated and designated case management tool accurately reflecting collaborative care planning, interventions and evaluation against defined targets and goals.
Triage patient or caregiver calls and coordinate timely care with the PCP or Comprehensive Care provider.
Manage Epic in-basket messages to ensure timely response to patient care needs.

QUALIFICATIONS: Education, Licensures, Certifications & Experience
Required: 5+ years of clinical and case management experience in a hospital, home health/hospice, geriatrics, or managed care setting
Required: Active Ohio RN license
Preferred: CCM certification; bachelor's degree in nursing, or CHPN certification
Knowledge, Skills & Abilities
Work well under pressure with the ability to assess, communicate and maintain composure in stressful situations.
Must be able to work flexible hours as needed
Working knowledge of HIPAA requirements and track record of maintaining patient confidentiality
Knowledge of chronic and acute disease states in adults
Understanding of common medications used in a primary care setting including indications, dosages, and side effects
Knowledge of trends in healthcare, managed care, government payers, case management, medical management, and quality improvement
Proficient in computer software and usage including but not limited to Microsoft Office (Word, Excel, Outlook, Teams) and EHR systems (Epic preferred)
Strong analytical, organizational, and time management skills
Capability to work independently and within a team environment with minimum supervision
Ability to demonstrate work toward the progress of patient-centered goals
Aptitude to develop and maintain rapport among health care professionals within individual COPC practices
Ability to adapt to changing environment
Self-disciplined, energetic, passionate, and innovative
Proficiency in decision making and problem-solving skills while showing great attention to detail
