At Starling Physicians, we believe quality care should start from the moment a patient is referred to us. Our Cardiology Patient Care Coordinator is a customer-oriented service professional responsible for coordinating and scheduling initial consultations with our cardiologists. They serve as an important first point of contact for new patients and referring offices, helping to ensure a seamless referral process and timely access to cardiology care. The Cardiology PCC processes referrals received through the EHR, fax, email, and telephone and is responsible for managing each referral from initial receipt through completion. This includes reviewing referral information for completeness, obtaining pertinent medical records, communicating with referring offices and patients, and facilitating timely and appropriate scheduling within the Cardiology Department.

This position requires strong organizational skills, attention to detail, effective communication, and the ability to independently manage a high-volume referral workload while supporting timely patient access to cardiology services.
Essential Functions/Responsibilities
- Responsible for managing a high volume of new patient cardiology referrals received through the Epic referral work queue, fax, email, and telephone, processing more than 400 referrals per month.
- Collaborate with cardiology providers, clinical staff, scheduling teams, referring offices, and other departments to facilitate timely and appropriate patient access.
- Request and obtain pertinent medical records from referring physicians, hospitals, healthcare systems, and other outside facilities to ensure necessary clinical information is available prior to the patient's cardiology visit.
- Review incoming referrals for completeness and follow up with referring offices and/or patients when additional information or documentation is required.
- Ensure pertinent clinical documentation is obtained, accurately indexed, and available within the patient's medical record to support continuity of care and appropriate cardiology evaluation.
- Coordinate with referring practices and internal cardiology teams to resolve referral-related issues and barriers that could delay patient care.
- Respond to patient and referring-office inquiries regarding the status of new patient referrals and scheduling in accordance with established departmental workflows and procedures.
- Ensure all referral-related communications and actions are accurately documented in Epic and work to resolve referral needs efficiently whenever possible.
- Provides monthly volume reports to management and leadership.
- Ability to multi-task and effectively utilize Epic and other systems while simultaneously managing referrals, medical-record requests, communications, scheduling needs, and follow-up activities.
- Communicate professionally with patients, referring providers, physician offices, and internal staff regarding referral requirements, outstanding documentation, and scheduling needs.
- Support new patient access across Cardiology practice locations by coordinating referrals with appropriate providers and locations in accordance with departmental scheduling guidelines and patient needs.
- Adhere to all HIPAA guidelines and policies.
- Comply with departmental goals, established referral workflows, productivity expectations, and quality standards.
Qualifications
- A High School Degree or GED
- 3+ years of experience in a medical office or health care setting
- Comfort with technology including the Microsoft suite of products
- Prior experience using an electronic health record (EHR); Epic experience strongly preferred.
- Passionate about customer service and patient satisfaction
- Ability to independently manage a high-volume workload with multiple referrals and competing priorities while maintaining accuracy and timely follow-through.

- Strong communication, interpersonal, and organizational skills
