Job Summary
The PACE LPN provides general nursing care to PACE participants, executes nursing interventions under the supervision of the PACE primary care providers and nursing leadership staff, and evaluates and triages participant medical conditions in the PACE clinic and by telephone, following established protocols and standing orders, and escalates findings to the RN or primary care provider for clinical assessment and decision-making.
The PACE LPN reinforces nursing education with participants, caregivers, and PACE staff based on teaching plans established by the RN or primary care provider and is responsible for scheduling participant appointments for PACE Clinic referrals made to medical specialists and ancillary care providers in the community.
Qualifications
Minimum Qualifications
- Current LPN license in good standing to practice in Georgia (CMS required).
- One year of experience working with a frail or elderly population or, in the absence of such experience, receive appropriate training from PACE on working with a frail or elderly population before providing direct patient care to PACE participants independently. Relevant experience must be within the last 3 years. (CMS required)
- Demonstrated competency in venipuncture and blood specimen collection, including proper specimen handling, labeling, and processing.
- Meet a standardized set of competencies for the specific position description established by PACE before working independently. (CMS required)
- Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines. (CMS required)
- Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act. (CMS required)
- Have not been excluded from participating in Medicare and Medicaid programs. (CMS required)
- Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse. (CMS required)
Competencies, Preferred Knowledge, Skills, and Abilities
- Minimum 3 years LPN clinical experience. (Preferred)
- Experience working in Electronic Health Records (EHRs) (Preferred).
- BLS certified (Preferred).
- Strong LPN clinical knowledge with the ability to perform nursing interventions, administer medications safely, and triage participant needs independently, escalating concerns appropriately.
- Knowledge of the clinical and psychosocial needs of a frail or elderly population, including the ability to recognize and respond to changes in participant status.
- Strong care coordination and case management skills, particularly during care transitions, with the ability to collaborate effectively within an Interdisciplinary Team (IDT).
- Proficiency with Electronic Health Records (EHRs) and knowledge of PACE regulatory and organizational documentation requirements.
- High degree of accountability, sound independent clinical judgment, and strong organizational skills with the ability to manage multiple priorities and frequent interruptions.
Physical Demands and Work Environment
This position requires moderate to intense visual concentration, including frequent reading, computer use, and detailed review of patient charts. The role involves daily interaction with patients, families, medical facilities, physicians, physician office staff, vendors and suppliers, and Hospice Savannah staff, and therefore demands a consistently professional demeanor, strong communication skills, and effective problem-solving abilities. The employee may be exposed to infectious diseases and must demonstrate competence in the use of standard safety precautions. The position also requires sufficient mobility to ambulate or lift patients, with or without assistive devices, in excess of 75 pounds while using proper body mechanics. In addition, the employee regularly encounters emotionally stressful situations related to end-of-life care and may be required to provide services in high-crime or potentially unsafe environments.
Position Type and Expected Hours of Work
This is a full-time, non-exempt position working 40 hours per workweek, rotating days, evenings, or weekends, with occasional requests to work additional days/hours/holidays as needed. Participates in the nursing on-call rotation to provide after-hours and weekend call coverage to PACE participants.
