Schedule Details/Additional Information:
Ability to travel within the metro community. Frequent need to attend off-site meetings. Able to work on evenings or weekends in community as needed.
Pay Range:
$26.55 - $39.85
Major Responsibilities:
- Coordinates and implements evidence-based community health programing that support needs identified at designated hospital sites and prioritizes collaboration with community partners.
- Coordinates and implements specific tactics associated with these programs such as attending meetings, ordering supplies, gathering data, coordinating events and screenings and providing support to community partners.
- Collects monitoring and outcome data associated with the various community health programs.
- Participates in site Community Health Council meetings and other site meetings that promote system alignment with community health strategies.
- Collaborates with the director to implement data collection activities related to the Community Health Assessment to enable each hospital to measure community health needs and the impact of hospital interventions.
- Educates associates, management and hospital governance on issues impacting the health of the community and on the hospital’s planned interventions to impact these issues.
- Implements activities to ensure accurate collection and summary of community benefit data for designated hospital sites.
- Facilitates reporting of community benefit activity for the hospital to ensure accuracy and monitor on-going hospital community benefit reporting.
- Supports and maintains trusting relationships with community leaders/representatives, faith groups, community based organizations, political/government officials and non-Advocate health care organizations/hospitals as well as other Advocate hospitals to address community needs.
- Represents designated hospital sites in local coalitions with a focus on coalitions addressing community health needs.
- Communicates site CHNA findings to community groups and solicit involvement in addressing identified needs.
Education Required:
- High School Diploma or GED
Licensure, Registration, and/or Certification Required:
- N/A
Experience Required:
- 2 years of experience addressing and improving the social drivers of health, community health strategy, population health initiatives, or related areas.
Knowledge, Skills & Abilities Required:
- Strong analytical and writing skills.
- Knowledge of current and emerging best practices in large health systems addressing non-clinical drivers of health.
- Ability to coordinate community health programs, partnerships, meetings, events and related implementation activities across internal and external stakeholders.
- Ability to collect, monitor and summarize program, community health assessment and community benefit data to support reporting, compliance and continuous improvement.
- Computer literacy in Microsoft programs including Excel, Publisher, Word and Access.

Preferred Education:
- Bachelors degree in public health, epidemiology, social work, community development, public policy or a related field addressing the social drivers of health.
