Agape has current openings for a Family Connector.

The Family Connector advances Agape's mission through a hope-centered, two-generation (2Gen) approach that promotes family stability, well-being, and human flourishing. Working within schools, neighborhoods, and communities, the Family Connector builds authentic relationships with children, youth, caregivers, and families while providing individualized support, resource navigation, advocacy, and coordinated interventions designed to reduce barriers and strengthen opportunities for long-term success.

Through Agape's place-based strategy and Powerlines Community Network (PCN), the Family Connector partners with families to identify strengths, cultivate hope, and create pathways toward economic mobility, educational achievement, healthy relationships, and overall well-being. This role empowers families to build agency, develop actionable goals, and access resources that support sustainable growth and flourishing across generations.

Operating within a trauma-informed, resilience-building, and strengths-based framework, the Family Connector collaborates with families, schools, community partners, and internal teams to ensure high-quality service delivery, accurate documentation, meaningful outcomes, and continuous quality improvement.

This role is primarily community-based and may require a flexible schedule, including evenings and weekends.

RESPONSIBILITIES AND DUTIES:

Family Engagement & Support

- Build trusting, strengths-based relationships with children, youth, caregivers, and families through consistent engagement and meaningful support.

- Conduct individualized and family-centered assessments that identify strengths, aspirations, challenges, and opportunities.

- Develop and monitor individualized service plans aligned with family voice, choice, goals, and priorities.

- Provide ongoing support, coaching, encouragement, and advocacy to help families achieve desired outcomes.

- Maintain regular contact with families according to program standards, including home, school, office, and community-based visits.

- Respond appropriately to family crises and emerging needs through intervention, resource coordination, and referrals.

Resource Navigation & Stability Planning

- Connect families to educational, workforce development, mental health, healthcare, housing, parenting, substance abuse, financial capability, and other supportive services.

- Coordinate and monitor services to ensure timely access and continuity of care.

- Support families in overcoming barriers that impact stability, self-sufficiency, and well-being.

- Facilitate activities, workshops, and events that strengthen family engagement and promote goal attainment.

- Collaborate with community agencies and partners to maximize available resources and opportunities for participating families.

Hope-Centered Practice & Human Flourishing

- Apply hope-centered principles that strengthen agency, pathways, and willpower among children, youth, and families.

- Foster environments where families can envision positive futures and identify actionable steps toward their goals.

- Support participants in recognizing and building upon their strengths, resilience, and existing assets.

- Promote human flourishing through relationship-centered engagement that advances belonging, purpose, well-being, connection, and opportunity.

- Utilize trauma-informed and resilience-building practices that support healing, growth, and empowerment.

- Encourage family-driven decision-making and active participation in service planning and goal achievement.

Community Collaboration & Advocacy

- Build and maintain collaborative relationships with schools, community organizations, faith-based partners, healthcare providers, and other stakeholders.

- Work in partnership with Agape departments and Powerlines Community Network partners to ensure coordinated support for families.

- Represent Agape professionally at meetings, trainings, community events, and collaborative initiatives.

- Serve as an advocate for children, families, and the communities Agape serves.

- Promote awareness of available resources and opportunities that support family success and community well-being.

Data, Documentation & Continuous Quality Improvement

- Maintain accurate, timely, and compliant case documentation within Agape's case management systems, including CaseWorthy as applicable.

- Track participant engagement, service delivery, goal progress, and outcomes in accordance with program requirements and grant standards.

- Utilize data to inform service planning, participant support strategies, and program improvement efforts.

- Prepare and submit reports, referrals, assessments, and required documentation within established timelines.

- Participate actively in Agape's Performance Quality Improvement (PQI) processes and standards of excellence.

- Support program transparency, accountability, and outcome measurement through accurate data collection and reporting.

Organizational Engagement

- Participate actively in team meetings, supervision, training, and professional development opportunities.

- Attend multidisciplinary team meetings, agency trainings, and community engagement activities as assigned.

- Maintain positive and effective communication with Agape staff, participants, and community stakeholders.

- Support organizational priorities aligned with Agape's mission, values, and strategic direction.

- During summer programming periods, assist with youth and family engagement initiatives as assigned.

- Perform other duties within the general scope of the role as assigned by leadership.

Physical Requirements:

- Prolonged periods of sitting at a desk and working at a computer.

- Prolonged walking, standing, and climbing stairs within schools and community sites, and during community events.

- Must be able to lift up to 20 pounds at a time.

Qualifications

Required

- Bachelor's degree in Social Work, Education, Human Services, Psychology, or a related field from an accredited college or university; or an equivalent combination of education and experience.

- Minimum of two (2) years of relevant experience in case management, family support, community-based services, social services, education, or related fields.

- Demonstrated experience working with diverse populations and delivering culturally responsive services.

- Strong written and verbal communication skills.

- Proficiency in Microsoft Office 365 applications and the ability to learn additional organizational platforms and systems.

- Experience utilizing case management systems and maintaining accurate participant documentation.

- Strong organizational, problem-solving, and time management skills.

- Understanding of data collection, reporting requirements, and outcome measurement.

Preferred

- Master's degree in Social Work or a related field.

- Experience working within a Two-Generation (2Gen), family-centered, or place-based service model.

- Experience using CaseWorthy or similar case management platforms.

- Experience implementing hope-centered, strengths-based, trauma-informed, or resilience-building approaches.

- Knowledge of community resources, family systems, and social service networks.

Additional responsibilities include flexibility for evening and weekend work as needed, reliable transportation for business use, including a current driver's license and current auto insurance with acceptable coverage levels, and engagement in Agape's Performance Quality Improvement standards of quality and excellence.
