Position Summary

The Clinical Pharmacy Specialist provides advanced, patient-centered clinical pharmacy services with a focus on medication optimization, disease state management, specialty pharmacy practice, and interdisciplinary collaboration. This role practices at the top of licensure under statewide protocols and WellPower Collaborative Practice Agreements (CPAs), while supporting clinical innovation, program development, and improved patient outcomes. The Clinical Pharmacy Specialist serves as a subject matter expert in clinical pharmacy practice and provides comprehensive medication management services for behavioral health conditions, substance use disorders, diabetes, metabolic disorders, ambulatory care, infectious disease prevention and treatment, population health, and other chronic disease states consistent with organizational programs, practitioner competency, and scope of practice. The position provides support to current and future initiatives as outlined by pharmacy and WellPower leadership.

The posted range represents the full hiring range. Offers will not exceed this range, the majority of candidates are placed near the midpoint based on experience and qualifications.

Job Duties & Functions:

- Advanced Clinical Care & Consultation

- Design and optimize individualized pharmacotherapy plans

- Conduct complex clinical consultations within area(s) of specialty practice.

- Identify and mitigate medication-related risks, including drug interactions and adverse events, and barriers to adherence.

- Provide timely, evidence-based drug information to providers and interdisciplinary teams

- Document clinical recommendations, interventions, and patient outcomes

- Provide pharmacist-delivered, protocol-driven clinical services under Collaborative Practice Agreements (CPAs), including initiating, modifying, and monitoring therapy as authorized.

- Assess, initiate, monitor, and optimize medication regimens and clinical outcomes for patients with behavioral health conditions, diabetes, metabolic disorders, and other chronic disease states as appropriate to specialty area.

- Administer medications and clinical therapies within scope of practice and organizational protocols, including long-acting injectable therapies, immunizations, and other pharmacist-delivered services.

- Apply and interpret statewide and organizational clinical protocols to guide patient care and decision-making

- Program Development & Clinical Leadership

- Support implementation, evaluation, and continuous improvement of:

- Collaborative Practice Agreements

- Statewide protocols

- Specialty clinical pharmacy services

- Disease-state management programs

- Develop and support clinical programs, medication use guidelines, and population health initiatives

- Identify opportunities to expand pharmacist-delivered services and protocol-driven care

- Partner with pharmacy and medical leadership to design and enhance clinical service models

- Participate in committees, work groups, and organizational initiatives, as directed

- Provide support to current and future initiatives as outlined by pharmacy and WellPower leadership.

- Education & Workforce Development

- Serve as a preceptor for pharmacy students, residents, and other learners as appliable.

- Provide education to pharmacists, providers, nurses, and interdisciplinary care teams

- Act as a clinical resource for pharmacy and medical staff within area of expertise.

- Promote evidence-based practice and continuous professional development

- Provide patient education focused on medications, disease management, self-care strategies, and health promotion.

- Quality, Safety & Outcomes

- Participate in medication safety programs, including adverse drug reaction and medication error monitoring

- Support quality improvement initiatives tied to clinical and operational outcomes

- Contribute to the development, monitoring, and reporting of clinical program metrics such as interventions, adherence, clinical outcomes, quality measures, and cost impact.

- Utilize data to identify opportunities for improving patient care and service delivery.

- Integrated Pharmacy Practice

- Perform pharmacist duties as needed, including prescription verification and dispensing

- Support technician workflow during operational needs

- Provide MTM services and administer immunizations per protocol.

- Collaborate with medical, behavioral health, nursing, and care coordination teams to deliver integrated patient care.

- Ensure compliance with regulatory, legal, accreditation, and professional standards

- Patient Experience & Equity

- Deliver culturally responsive, trauma-informed care

- Provide individualized medication education to support adherence, self-management, and patient engagement

- Support equitable access to medications, clinical services, and preventative healthcare.

- Role Flexibility & Scope

- Adapts clinical programs and services in response to evolving regulatory standards, organizational priorities, and patient care needs

- Participates in the implementation of new services, protocols, and care delivery models consistent with pharmacist licensure, specialty training, and organizational agreements

- Maintain competency in specialty practice areas and applicable clinical certifications.

- Other duties as assigned

Requirements & Qualifications:

Education & Licensure

- Doctor of Pharmacy degree required.

- Advanced clinical training through residency, board certification, certificate programs, diabetes education training, or equivalent clinical experience preferred.

Preferred Certifications (Specialty-Specific)

One or more of the following preferred:

- Board Certified Psychiatric Pharmacist (BCPP)

- Board Certified Ambulatory Care Pharmacist (BCACP)

- Board Certified Pharmacotherapy Specialist (BCPS)

- Certified Diabetes Care and Education Specialist (CDCES)

- Other specialty certification relevant to assigned clinical practice area

Experience

- LICENSURE: Colorado Pharmacist license in good standing.
