Job Summary
The Psychiatric Rehab Services Assistant is responsible for monitoring the front entrance, answering the main phone line and serving as behavior management specialist within the social service department.
Job Description
- Monitor the front entrance of the building. Serve as the first point of contact for all visitors, agencies, families and residents.
Monitor residents community access pass program in accordance with the facilitys policy . Keep accurate sign-in/sign-out sheets for all residents entering and exiting the building.
- Notify charge nurse if a resident on community pass is gone over allotted time.
- Remind residents to dress appropriately for weather conditions.
- Inspect personal items brought into the facility; if inappropriate contraband is found, give to department head or charge nurse.
- Ask visitors to sign in the visitor log . Check IDs as relevant to people visiting residents until you know the person.
- Direct all visitors in a professional manner and assist with requests.
- Former employees are required to obtain prior approval to be on the premises.

- Transfer calls appropriately. If you are not sure about where a call goes, put the call on hold and ask a department head or nurse. If a call comes back to the line, take a message and direct it appropriately. Do NOT keep transferring call if there is no one to pick up the phone.
- Take paper messages as appropriate. Be sure to fill in ALL areas on message forms (especially date, time, return phone number, who call is for and ask a reason for the call).
- Assist with alphabetizing and filing needs while at the front desk.
- Monitor the front desk area and remain at the desk at all times.
If paged for behavior management, find another staff to cover the front OR lock the door to ensure it is not left unattended.
- Page a housekeeper, CNA or designated staff to fill in for 15 minute breaks, lunch breaks and/or assigned duty.
- Remain awake and alert for duration of shift.
- Keep front desk area clean and presentable at all times.
- Make sure other staff do not loiter at the front desk area.
- Assist in enforcing employee schedules. Employees who are off duty are not encouraged to be in the facility until the start of the shift or unless they have received prior permission from a department head.
- Assist in ensuring staff are appropriately using the time clock.
- Perform rounds of the building (for no more than 10 minutes in conjunction with break times) approximately every two hours each shift. Outside perimeter check should be performed once a shift in conjunction with lunch break.
- First shift staff will perform routing between the hours of 9am – Noon. At times other deliveries and equipment may need to be transported across campus as assigned.
- Monitor resident behaviors and provide behavior management interventions as appropriate. Document behaviors using appropriate forms.
- Assist patients with daily living tasks (IE: passing cigarettes and monitoring supervised smokers, assisting patients with transportation-getting in and out of vehicles if needed), assisting in the dining room passing trays/clearing trays/getting needed items. 2 nd and 3 rd shift staff may be required to assist the facility in rolling cigarettes for patients.
- Maintain confidentiality for facility business purposes and patient HIPAA privacy.
- Complete assigned paperwork in a timely manner which includes but is not limited to resident sign-in/out sheets, shift reports, behavior observation forms, phone messages, and other duties as assigned.
- Attend all staff meetings and in-service trainings as scheduled.
- Perform other duties as assigned.
