We're looking for a Medical Social Worker (MSW) to provide psychosocial support to home health and hospice patients and families throughout Salt Lake County.

This is a part-time position with a flexible schedule , ideal for a social worker who values meaningful patient interaction, professional autonomy, and the ability to organize visits around patient needs.

Qualifications

Required:

- Master's degree in Social Work (MSW) from an accredited social work program

- Current Utah social work license in good standing (CSW or LCSW)

- At least 1 year of social work experience in a healthcare setting

- Strong assessment, communication, and interpersonal skills

- Ability to work independently in patients' homes

- Valid driver's license and reliable transportation

Preferred:

- Home health or hospice experience

- Palliative or end-of-life care experience

- Experience with grief, bereavement, serious illness, or community resources

About the Role

As a Medical Social Worker, you'll help patients and families navigate the emotional, social, financial, and practical challenges that can accompany illness, disability, and end-of-life care.

You'll work directly with patients and families in their homes while collaborating with nurses, therapists, physicians, chaplains, and other members of the interdisciplinary care team.

Responsibilities

- Complete psychosocial assessments for home health and hospice patients

- Identify emotional, social, financial, caregiver, and community-resource needs

- Provide counseling, education, and supportive interventions within your scope of practice

- Connect patients and families with community resources and available benefits

- Provide crisis intervention and support when appropriate

- Assist with advance-care planning and advance directives

- Assess caregiver stress, coping, grief, and bereavement needs

- Advocate for patients and families

- Participate in hospice interdisciplinary team meetings and care planning

- Assist with discharge planning and transitions of care

- Identify psychosocial, cultural, and spiritual concerns and coordinate additional support when appropriate

- Complete accurate and timely clinical documentation

Part-Time | Flexible Schedule

Patient visits are scheduled based on patient needs, with flexibility throughout the week.

Territory: Salt Lake County, Utah
Work Setting: Patient homes and residential care settings
