Find your purpose at CentraCare working as a Care Coordinator Registered Nurse or Licensed Social Worker! In this role, you will work with specific patient populations to support payer contract requirements and achieve identified quality outcomes. CentraCare is a delegate for Medica Minnesota Senior Health Options (MSHO) Care Coordination, and this position will provide care coordination for the Medica MSHO population.
As a Care Coordinator, you will:
- Coordinate the provisions of all health and long-term care services covered by the payer programs, including Medicare, Medicaid and waiver program services.
- Partner with patients, families, providers, payers and community resources to ensure appropriate services are in place.
- Serve as a liaison between the payer and health system to support seamless, patient-centered care.
- Assess eligibility and identify services for patients with long-term care needs, including nursing home placement when appropriate.
- Advocate for patients and collaborate with care teams to promote quality outcomes and meet payer requirements.
Schedule:
- Full-time | 80 hours every two weeks
- Monday - Friday | Day shift | 8:00 am - 4:30 pm
- This position primarily works from home but does include traveling to patients’ homes and care facilities for in-person visits as well as quarterly on-site team meetings.
Pay and Benefits:
- Pay based on experience and licensure: RN pay range: $92,539.20 - $138,860.80 annually
- Licensed Social Worker pay range: $67,641.60 - $101,483.20 annually
- $3,500 (RN) and $3,000 (Social Worker) hiring bonus

Qualifications:
Registered Nurse
- Graduate of an accredited school of nursing with a Bachelor’s Degree or higher in nursing
- Current licensure as a Registered Nurse (RN) in the State of Minnesota
- Minimum of two years’ experience as a Registered Nurse
- Certified MnChoice Assessor within 30 days of employment
- Current American Heart Association (AHA) BLS Healthcare Provider card
- Valid Minnesota driver’s license, a driving record in good standing, reliable transportation and insurance liability coverage meeting CentraCare’s standards required
OR
Licensed Social Worker
- Bachelor’s Degree in Social Work
- Licensed Social Worker (LSW) in the state of Minnesota
- Certified MnChoice Assessor within 30 days of employment
- Minimum of two years’ experience as a Social Worker
- Valid Minnesota driver’s license, a driving record in good standing, reliable transportation and insurance liability coverage meeting CentraCare’s standards required
Preferred Qualifications:
- Experience in working with seniors, those with disabilities and mental health
- Experience with Medical Assistance and Medicare including home care community based waiver programs
- Understanding of the services and needs of the community preferred.
