Physical Therapist (PT) – Mobile Outpatient | West Palm Beach, FL | PRN
Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to provide individualized, one-on-one mobile outpatient therapy to patients throughout West Palm Beach, FL, and surrounding communities .
This opportunity is ideal for a Physical Therapist looking to supplement an existing schedule, maintain a small caseload, or add flexible patient visits while maintaining control over when and where they work.
With a minimum commitment of just 5 patients , our PRN model provides flexibility while allowing therapists to build a geographically concentrated caseload.
What You'll Do
As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat patients one-on-one in their residences through our mobile outpatient model.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments.
- Help patients improve strength, balance, mobility, endurance, and functional independence.
- Provide gait training, transfer training, therapeutic exercise, and fall-prevention interventions.
- Recommend assistive devices and adaptive equipment when appropriate.
- Educate patients and caregivers on exercises, mobility strategies, and safety.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, and interdisciplinary care team members.
- Coordinate with Stern's administrative team regarding scheduling and patient care.
Compensation
PRN 1099 – Minimum 5 Patients
- $72 per follow-up visit
- $85 per evaluation
- Fixed per-visit compensation with no tiered volume model
- PRN positions are 1099 independent contractor roles only

Qualifications
- Active Physical Therapist license in Florida required
- Graduate of an accredited Physical Therapy program
- Reliable transportation required
- Strong clinical, communication, documentation, and time-management skills
- Ability to work independently in a mobile outpatient setting
- Geriatric or mobile outpatient experience preferred but not required
