JOB DESCRIPTION:

SUMMARY OF POSITION - Intern

Masters Level intern (social work/counseling), will work collaboratively with the discharge planner and CSU/WIC teams to develop comprehensive discharge plans for Clients admitted into the CSU.

Communicates clearly and appropriately; provides excellent customer service. Adheres to applicable policies and procedures. Regular attendance and punctuality is required.

ESSENTIAL DUTIES & RESPONSIBILITIES

Meets one-on-one with each Client in CSU/WIC to collaboratively develop a discharge plan that addresses that Client’s presenting problem/primary needs:

Mental health: Outpatient appointment scheduling (psychiatric/counseling), safety planning, restricting access to lethal means, connection to follow-up programs

Physical health: PCP resources and appointments as needed

Substance Use: Relapse prevention plan, treatment resources and referrals as needed

Housing: Resources and referral assistance as needed

Financial, Legal, Social: Resources and referrals as needed

KNOWLEDGE, SKILLS & ABILITIES

Effectively communicate via written, verbal, in person and virtual methods.

Perform basic electronic medical record computer skills

Detail oriented with effective problem solving skills

Knowledge base in community resources

Skill in crisis intervention

Ability to de-escalate patient/clients using evidence based de-escalation training (e.g. Theapeutic Options) to include safety holds as necessary.

QUALIFICATIONS

Education Level

Bachelor's degree required. Enrolled in a Masters Level Program.
