Job Summary
This combined position is responsible for providing direct clinical care as a Clinical Nurse Practitioner while serving as the NP Program Manager for the Nantucket Tick Center.

Approximately 50% of the position will be dedicated to Clinical Nurse Practitioner responsibilities and 50% to the development, implementation, operation, and growth of the Nantucket Tick Center.

The role combines direct patient care with clinical program leadership, evidence-based practice, patient navigation, education, quality improvement, research coordination, and program development

Qualifications

Clinical Nurse Practitioner (50%)

The Clinical Nurse Practitioner will:

- Provide direct patient care consistent with licensure, scope of practice, credentialing, and NCH clinical standards.
- Evaluate and manage patients within the assigned clinical practice.
- Collaborate with physicians, advanced practice providers, nurses, and other members of the care team.
- Coordinate diagnostic testing, referrals, treatment, and follow-up.
- Support evidence-based clinical practice, quality improvement, and patient safety.
- Provide patient education and support continuity of care.
- Maintain timely and accurate clinical documentation.
- Participate in the development and implementation of clinical protocols and workflows, as appropriate.
- Strong clinical knowledge, evidence-based practice, communication skills, and five to seven years of Nurse Practitioner experience.

NP Program Manager, Nantucket Tick Center (50%)

The NP Program Manager will:

- Lead the day-to-day development and operation of the Tick Center.
- Lead the clinical and operational implementation of the Tick Center.
- Coordinate patient intake, referrals, triage, testing, consultations, specialty care, telehealth, and follow-up.
- Support patient navigation and continuity of care between NCH and off-island specialists.
- Develop and maintain practical clinical workflows and referral pathways for Lyme disease, babesiosis, anaplasmosis, ehrlichiosis, alpha-gal syndrome, and other tick-borne or tick-associated conditions relevant to Nantucket.
- Coordinate interdisciplinary care and facilitate communication among clinicians and specialty partners.
- Working with the team, develop program policies, protocols, workflows, goals, timelines, and performance measures.
- Serve as an operational liaison with Mass General Brigham, Massachusetts General Hospital, the Morse Clinic, Martha's Vineyard Hospital, researchers, public health partners, and community organizations.
- Coordinate clinician, patient, and community education and prevention initiatives.
- Support the development of multidisciplinary clinical services for patients with complicated disease, persistent symptoms, or specialty care needs.
- Support clinical and prevention research, including study operations, participant recruitment, data collection, regulatory processes, and coordination with investigators.
- Track program activity, patient access, follow-up, quality measures, patient experience, education, and other outcomes.
- Prepare program updates and reports for NCH leadership.
- Identify operational barriers and develop practical solutions to improve access, coordination, and program performance.

These responsibilities reflect the existing Tick Center position's emphasis on clinical operations, patient navigation, education, interdisciplinary care, research, and measurable program outcomes.

Note: The position will not serve as a Principal Investigator unless the individual is appropriately qualified and formally appointed to that responsibility.
