Key Responsibilities

- Student Case Management: Manage an assigned caseload of students, ensuring individual needs, concerns, and support requirements are identified and appropriately addressed.
- Student & Family Support: Serve as a primary point of contact for students and families, providing guidance, follow-up, and support throughout the case management process.
- Case Coordination: Coordinate with academic, pastoral, counseling, learning support, and other relevant teams to ensure students receive appropriate and timely support.
- Individual Support Plans: Support the development, implementation, and monitoring of individualized action or support plans based on student needs.
- Monitoring & Follow-Up: Regularly monitor student progress, attendance, engagement, and identified areas of concern, ensuring appropriate follow-up and escalation when necessary.
- Communication: Maintain clear, timely, and professional communication with students, families, teachers, and internal stakeholders regarding student cases and agreed actions.
- Documentation & Records: Maintain accurate, confidential, and up-to-date case records, documentation, intervention plans, and follow-up notes in accordance with school procedures.
- Early Identification & Intervention: Identify emerging academic, social, behavioral, or wellbeing concerns and coordinate appropriate interventions and referrals.
- Referrals & External Support: Facilitate referrals to internal or external specialists and services when additional support is required, and monitor follow-through.
- Collaboration: Work closely with relevant school teams to ensure a coordinated and student-centered approach to case management.
- Confidentiality & Safeguarding: Handle sensitive student and family information with discretion and in accordance with school policies, safeguarding procedures, and applicable requirements.
- Reporting & Data: Maintain relevant case management data and provide updates and reports to appropriate leadership regarding caseloads, trends, interventions, and outcomes.
- Transition & Continuity of Care: Ensure effective handovers and continuity of support when students transition between grades, campuses, programs, or case managers.
- Continuous Improvement: Contribute to the review and improvement of case management processes, resources, and student support practices.

Skills, Qualifications and Experience

Skills:

- Strong knowledge of curriculum, pedagogy, and assessment.
- Ability to analyse and interpret student data to inform practice.
- Coaching, mentoring, and professional learning facilitation skills.
- Strong collaboration and leadership skills.
- Excellent communication and interpersonal skills.
- Problem-solving and reflective practice skills.
- Strong organisational and time-management skills.
- Knowledge of inclusive education and intervention strategies.

Qualifications:

- Bachelor´s Degree in Psychology or equivalent (required)

Experience:

- Demonstrated experience as an effective classroom teacher.
- Experience supporting teachers through coaching, mentoring, or professional learning.
- Experience implementing evidence-based intervention programs.
- Experience working with diverse learners, including students with additional learning needs.
- Experience contributing to whole-school improvement initiatives (desirable).
