About this role
Corewell Health in St Joseph and Niles, MI, is seeking an experienced, compassionate, Board Certified Physician to join the dedicated team of Hospice providers to help meet the needs of hospice patients in our community.
This position will be responsible for clinical oversight of a large hospice team, ensuring patients are appropriate for hospice, adhering to all regulations and CMS requirements for both the physician and the hospice service. Seeing patients as needed for acute symptoms, goals of care, or family meetings. Regularly assess patients for correct level of care while serving patients of Corewell Health Hospice of Southwest Michigan in the Allegan, Berrien, Cass, & Van Buren County service areas.
Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six months or less, serving individuals in their home, nursing facilities, and Hanson Hospice Center.
This is a full-time opportunity, Monday - Friday, 8am - 5pm, plus call coverage, for Corewell Health West and Corewell Health South.
We desire candidates who are fellowship trained and Board Certified in Hospice Care with 5 years of experience, who enjoy being part of a collaborative care team that includes APPs, nurses, therapists, care managers and spiritual caregivers.
Essential Functions
- Assesses and monitors the medical needs of patients/significant others.
- Provides quality medical services to patients in a cost effective, appropriate manner and ensures coverage of case load at all times.
- Provides input into the development of policies and procedures as they relate to medical practice and quality patient care.
- Provides oversight of Physician Assistants and Nurse Practitioners and others providing delegated services to patients.
- Evaluates patients'/significant others'/staff educational needs and develops and implements appropriate teaching strategies or delegates to appropriate resources.
- Documents in a manner consistent with regulatory standards.
- Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.
- Participates in ongoing process improvement activities through the development/application of clinical paths, process improvement and other related activities.
- Actively works with Corewell Health to meet financial and patient satisfaction goals.
Qualifications
- LIC-Physician (MD) - STATE_MI State of Michigan Upon Hire Required Or
- LIC-Osteopathic Physician (DO) - STATE_MI State of Michigan Upon Hire Required
- CRT-DEA Registration - STATE_MI State of Michigan Upon Hire Required
- CRT-Controlled Substance (CDS) - STATE_MI State of Michigan - Upon Hire Required
- Board Certification. Required
- At least four years' experience in clinical practice, preferably in a medical group setting. Required
- Experience in leading project teams and a commitment to develop as a physician leader. Preferred
