As a Spiritual Coordinator, you’ll contribute to our success in the following ways:
- Conduct initial and ongoing spiritual assessments to identify the needs, beliefs, and preferences of hospice patients and families.
- Develop individualized spiritual care plans and collaborate with the interdisciplinary team to integrate spiritual needs into overall patient care.
- Provide spiritual guidance, counseling, and emotional support while respecting each patient’s cultural and religious beliefs.
- Support patients and families with end-of-life concerns, including discussions around meaning, faith, forgiveness, and personal values.
- Provide bereavement support to families before and after a patient’s passing.
- Educate patients, families, and staff on spiritual care and maintain accurate, timely documentation of services provided.
What is Required?
- Bachelor’s or Master’s degree in divinity, Theology, Pastoral Care, or a related field
- 2 years of experience working at a hospice, counseling, or ministry environment
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
