About this role

SUMMARY__________________________________________________________________________

The Community Health Worker promotes patient care for MedLink Health Center(s) by providing care coordination/case management and helping eliminate barriers to ensure patients will achieve a higher level of health equity. They will facilitate patients in overcoming social determinants of health barriers that patients face, such as transportation, food, housing, etc. As a recognized patient-centered medical home (PCMH), every employee is responsible for ensuring the successful implementation of quality, comprehensive patient-centered care as a member of the Community Health Team. By performing the job expectations described below you will be an active member of the care team, thereby patients are assured access to team-based, coordinated, comprehensive, quality care.

ESSENTIAL DUTIES AND RESPONSIBILITIES ( include, but are not limited to, the following)

- Actively contact and reach out to all Medicaid patients to help prevent Medicaid Patients from being un-enrolled and will help assist those with re-enrollment activities.

- Assists patients with financial barriers and assists Medicaid recipients that may be facing re-enrollment challenges, as well as help align the patients' needs to other resources available.

- Routinely follows-up with patients to confirm patient has active insurance, proper nourishment, housing, transportation to and from medical appointments and overall, is ensuring the patient is receiving proper care.

- Answers all telephone calls promptly in a polite and professional manner.

- Answers questions and offers other information, as requested, to provide patient centered service and a positive impression of our organization.

- Maintains accurate and confidential patient records following HIPPA guidelines and regulations.

- Works with Chronic Care Management staff to refer patients as needed.

- Participate in team and staff meetings as required or requested.

- Other duties as assigned.

EDUCATION and/or EXPERIENCE__________________________________________________

- High School diploma or GED AND a minimum of one year related secretarial experience in the health care field or high school diploma (or equivalency) with three years general secretarial experience.

- Strong verbal and written communication skills

- Strong computer skills with Microsoft Excel experience preferred.

- Extremely dedicated in providing outstanding customer/patient service

- Excellent organizational skills and attention to detail
