Patient Care Coordinator / Staffing Coordinator
Mission Hills (Northridge Branch)

Pay Range:
LVN: $30-$35 per/hour
Non-Clinical: $28-$32 per/hour

What Youll Do

- Coordinate and maintain daily schedules for Home Health clinicians and patient visits

- Manage patient referrals and assign clinicians based on physician orders

- Process and prepare intake referrals accurately and efficiently

- Assist with nursing call schedules and staffing coordination

- Verify Medicare and insurance benefits

- Communicate with case managers and third-party payors as needed

- Support interdisciplinary care team meetings

- Maintain patient confidentiality and uphold HIPAA standards

- Provide excellent customer service to patients, families, referral sources, and staff

- Support agency goals, quality improvement initiatives, and daily operations

- Perform additional duties as assigned

What Were Looking For

- Minimum of 2 years of patient coordination, staffing, scheduling, or healthcare office experience

- Experience in Home Health, hospice, hospital, or medical office settings preferred

- Knowledge of Medicare guidelines and Conditions of Participation

- Strong organizational, multitasking, and communication skills

- Ability to work independently and collaboratively in a team environment

- Professional demeanor with strong customer service skills

- Reliable attendance and flexibility for occasional overtime

Physical Requirements

- Primarily sedentary work with occasional walking, standing, lifting, and stair climbing

- Ability to communicate effectively in English, both verbally and in writing

- Valid drivers license, proof of auto insurance, and safe driving record required
