About this role

We are seeking compassionate and skilled Licensed Practical Nurses (LPN) -Nursing Facility/ Registered Nurses (RN) Nursing Facility to provide high-quality care to residents within a nursing facility setting. You will be responsible for monitoring resident health, administering medications, and collaborating with a multidisciplinary team to ensure the best possible outcomes for those in your care.

Key Responsibilities

- Patient Monitoring: Observe and record residents’ conditions, vital signs, and medical histories.
- Medication Administration: Safely administer prescribed medications and treatments in accordance with facility protocols.
- Care Planning: (RN) Develop and implement comprehensive patient care plans; (LPN) Execute task-oriented duties as directed by supervisors.
- Daily Assistance: Assist residents with activities of daily living (ADLs), including feeding, grooming, and personal hygiene.
- Collaboration: Communicate effectively with physicians, family members, and nursing staff to coordinate treatments and assessments.

Required Credentials & Compliance

To ensure the safety and well-being of our residents, candidates must provide documentation for the following:

- Licensure: Active TX Nurse License (verified via Online License Verification).
- Registry: TX Nurse Aide Registry (NARS) status.
- Training: Completed CEU Dementia Training and signed Annual Mandatory Training Attestation Form.
- Health & Immunization: Influenza Vaccination.
- Physical exam (No expiration date).
- PPD results (2-step skin test or QuantiFERON); Positive PPD results require additional documentation.

- Background Checks: TX Criminal Offender Record Information (CORI) Search.
- State and National Sex Offender Registry Searches.

Skills & Qualifications

- Education: Graduate of an approved LPN or RN nursing program.
- Experience: Previous experience in a long-term care or nursing facility is preferred.
- Interpersonal Skills: Strong relationship-building skills and a consultative approach to resident care.
- Reliability: Self-motivated professional capable of working independently across various shifts.
