PRN Home Health & Therapy is seeking a dedicated Occupational Therapist to join our growing team. This position provides the freedom to manage your own schedule while making a lasting impact on patients throughout Monroe/South Madison.
In this role, you'll primarily provide care in a home health setting , with occasional visits to skilled nursing facilities. Whether you're an experienced clinician or a new graduate eager to begin a rewarding career in home health , this position offers an excellent opportunity to grow and succeed.
Key Responsibilities
- Work independently while maintaining strong communication with the clinical team.
- Review and respond to patient referrals in a timely manner.
- Collaborate with patients, caregivers, and interdisciplinary team members to develop individualized care plans.
- Create and implement treatment plans with measurable, patient-centered functional goals.
- Educate patients and caregivers to support successful outcomes and maximize independence.
- Provide guidance, direction, and supervision to Certified Occupational Therapy Assistants (COTAs).
- Assess and recommend durable medical equipment (DME), including wheelchairs, adaptive equipment, and other assistive devices.
- Partner with healthcare professionals to ensure coordinated, high-quality patient care.
- Perform additional duties as assigned.
