Physical Therapist Assistant (PTA) – Home Health
Status: 0.8 - 1.0 FTE
Location: North Madison & Surrounding Areas

Key Responsibilities
- Deliver Physical Therapy treatments according to the plan of care developed by the Physical Therapist
- Collaborate with the interdisciplinary team to identify and assist with obtaining patient‑specific DME (mobility aids, wheelchairs, cushions, orthotics, adaptive equipment, etc.)
- Maintain accurate daily treatment records and progress notes as directed by the Physical Therapist
- Work with the Physical Therapist and clinical team to educate patients and caregivers on safe discharge planning
Requirements
- Graduate of an accredited Physical Therapist Assistant program
- Current Wisconsin PTA license/certification
- Current American Heart Association CPR certification
