Affirmed Home Care is looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our team and provide high-quality one-on-one care to clients throughout New Brunswick and surrounding Central New Jersey areas .

What You’ll Do
- Provide compassionate nursing care based on the client’s individualized care plan
- Administer medications and prescribed treatments
- Monitor the client’s condition and communicate changes appropriately
- Provide wound, catheter, and other skilled nursing care within your scope of practice
- Maintain accurate and timely documentation
- Support the client’s comfort, safety, dignity, and independence
- Communicate professionally with the client, family, and care team
What We’re Looking For
- Active New Jersey LPN License
- 1+ year of nursing experience preferred
- Current CPR/BLS certification
- Strong communication and clinical skills
- Reliable, compassionate, and professional
- Comfortable working independently in a home care setting
- Valid driver’s license and reliable transportation preferred
- EHR/documentation experience preferred
