Affirmed Home Care, New York’s premier concierge home care agency, is looking for experienced Licensed Practical Nurses (LPNs) to provide exceptional one-on-one care to our clients throughout Suffolk County .
The Opportunity
We are seeking compassionate and dependable LPNs for highly specialized cases , with flexible 6–12 hour shifts available throughout Suffolk County.
Cases may be available in areas including:
- Huntington
- Smithtown
- Kings Park
- Surrounding Suffolk County communities
Compensation
- $40.00 per hour for hourly shifts
- Flexible per diem scheduling
- Overtime opportunities may be available

Requirements
- Minimum of 1 year of current LPN experience
- Home care experience is a plus
- Valid New York State LPN License
- Valid BLS Certification
- Valid driver’s license preferred
- Eligible to work in the United States
- Physical exam completed within the past year
- PPD or QuantiFERON completed within the past year, or chest X-ray within the past 5 years
- MMR immunizations within the past 10 years
