Schedule: Monday–Friday | 8:00 AM–10:00 PM ET

Position Overview
PharmD Live is seeking a detail-oriented, patient-focused Medical Assistant to support our telehealth care management program. This role combines patient outreach, appointment scheduling, care coordination, and core Medical Assistant support in a remote, high-volume environment.
Key Responsibilities
- Conduct outbound and inbound patient calls and schedule appointments
- Support Annual Wellness Visits and care management outreach
- Perform medication reconciliation and update patient information
- Document patient interactions accurately in the EHR
- Prepare charts and support care coordination and follow-up
- Maintain HIPAA compliance and escalate clinical concerns appropriately
- Meet established outreach, scheduling, and documentation targets
Qualifications
- Medical Assistant certification or equivalent healthcare experience required
- Prior healthcare, patient outreach, scheduling, or care coordination experience
- Strong communication, organization, and documentation skills
- Comfortable with high-volume calling and performance metrics
- EHR experience preferred
- Medicare patient experience preferred
- Reliable and self-motivated in a remote environment
