At Sunshine Home Health and Hospice, we are dedicated to providing compassionate, end-of-life care to patients and their families. We are currently seeking a Hospice Certified Nursing Assistant (CNA) to join our team and serve patients living in the Cache Valley area.
Position Overview:
As a Hospice Certified Nursing Assistant ( YOU MUST BE CERTIFIED FOR THIS POSITION) at Sunshine Home Health and Hospice, you will play a crucial role in providing exceptional care and support to terminally ill patients. You will work closely with a team of healthcare professionals to ensure that patients are comfortable, safe, and receive the highest quality of care during their final journey. Key Responsibilities:
- Provide compassionate and patient-centered care to hospice patients in their homes or in our hospice facility.
- Assist with activities of daily living, including bathing, dressing, grooming, and mobility.
- Administer medications and treatments as directed by the hospice nurse or medical team.
- Monitor and report changes in patient condition, symptoms, and vital signs.
- Offer emotional support and companionship to patients and their families.
- Ensure a safe, clean, and comfortable environment for patients.
- Collaborate with the hospice care team to develop and implement care plans.
- Maintain accurate and thorough documentation of patient care.
Qualifications:
- Current Certified Nursing Assistant (CNA) certification in the state of Utah.
- CPR and First Aid certification.
- High school diploma or equivalent.
- Must have a valid driver's license, proof of auto insurance, and reliable transportation.
- Ability to work with a diverse team in a dynamic, emotionally charged environment.
- Previous experience in hospice care or a healthcare setting is a plus.
