About this role

Job Purpose:
- The Care Coordinator functions as an advocate and support person to those individuals assigned. This employee links them to appropriate services and monitors the progress or lack of progress of the individual. The Care Coordinator works closely with the individual, the individuals family, LifeStream staff, and other community organizations to meet those needs and to allow the individual to live as independently as possible within the community.

Key Responsibilities:
- Convenes/attends staffing as necessary for assessing and coordinating services for clients.
- Direct care services include assessment interventions and case management.
- The ability to comply with the regulations requiring a workplace of dignity and respect and equal employment opportunity compliance.
- Services provided within the community and the team members must be flexible to meet with individuals at various times.
- Completes required agency, funding, legal, statistical and fiscal documentation in an accurate and timely manner.
- Participate consistently in all in-service training programs.

Qualifications

Required Education:
- Bachelors Degree with major course work in the human services field required

Required Experience:
- Minimum of Two years experience in the social services field
- Minimum of Two (2) years of experience working with the chronically mentally ill in an acute psychiatric or case/care management function required
- If in personal recovery, two (2) years of successive recovery required
- Minimum of 5-years driving experience required
- Safe Driving record required
- Must be over the age of 21

Required Skills and Abilities:
- Strong understanding of mental health and behavioral health concepts.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
- Ability to assess patient needs and develop individualized care plans.
- Proficiency in using electronic health records and other healthcare management software.
- Strong organizational skills with the ability to manage multiple cases and prioritize tasks effectively.
- Demonstrated ability to work independently and as part of a team in a fast-paced environment.
- Knowledge of community resources and ability to connect patients with appropriate services and support.
