Registered Nurse – Per Diem Assessments and  On-Call Visits:

Affirmed Home Care is seeking qualified and dependable Registered Nurses to perform per diem assessments and on-call nursing visits in patients’ homes. Serving all counties in North Jersey.

This opportunity is ideal for Registered Nurses interested in earning additional compensation while continuing in their current field-based or full-time position. Visits may be accepted according to the nurse’s availability.

Job Responsibilities:

- Complete start-of-care assessments
- Conduct supervisory nursing visits
- Perform patient reassessments
- Complete post-hospitalization visits
- Provide on-call nursing visits as assigned
- Assess patients’ clinical conditions and care needs
- Report changes in patients’ conditions to the clinical team
- Complete all required visit documentation accurately and on time
- Maintain patient confidentiality and follow HIPAA requirements
- Comply with Affirmed Home Care policies and applicable nursing regulations

Qualifications:

- Active and unrestricted Registered Nurse license
- Previous nursing experience required
- Home care or field nursing experience preferred
- Strong assessment and documentation skills
- Ability to work independently in patients’ homes
- Excellent communication and time-management skills
- Reliable transportation
- Current CPR or BLS certification
