Case Manager I Job Description The Case Manager I will work collaboratively with a multidisciplinary healthcare team to provide case management services to clients. This role involves conducting comprehensive bio-psychosocial culturally-sensitive assessments, synthesizing program goals, and creating individualized plans of care. Responsibilities

- Conduct comprehensive bio-psychosocial assessments with clients and their families.
- Create individualized plans of care based on assessed needs and program goals.
- Arrange and coordinate service referrals and provide ongoing monitoring of client progress.
- Educate clients and families about HIV/AIDS to optimize adherence to care.
- Maintain current knowledge of medical and psychosocial developments related to HIV/AIDS.
- Collaborate with case managers from other regions as needed.
- Manage a caseload by maintaining regular contact with clients and documenting activities.
- Perform indirect, non-clinical tasks such as data compilation and participation in training.
- Participate in multidisciplinary team meetings to ensure coordinated care delivery.

Essential Skills

- Bachelor’s degree in a health-related field such as social work, counseling, sociology, or psychology.
- Experience in case management, managed care, and care management.
- Strong problem-solving and organizational skills.
- Ability to work well with clients and maintain a customer service attitude.
- Creativity and the ability to work independently without much guidance.
- Experience with charting and paperwork management.

Work Environment The position is based in an office setting with a day shift from 8:30 am to 4:30 pm or 9 am to 5 pm, with one remote day per week. The team is collaborative, working well together, and an open-door policy is encouraged.
Job Type & Location
This is a Contract to Hire position based out of Hazelwood, MO.
Pay and Benefits
The pay range for this position is $21.00 - $25.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
