Psychiatric Nurse Practitioner (IMT)

Intro/Program Description: Goddard Riverside is a well-established Manhattan community-based agency with an annual operating budget of approximately $50 million. Goddard Riverside is one of New York Citys leading human service organizations meeting the basic needs of children, youth and families, homeless people, and older adults across Manhattan and New York City.

The Intensive Mobile Treatment (IMT) Team provides treatment and support services for individuals with frequent interaction with the mental health, substance abuse, criminal justice and homeless services systems and have been poorly served by traditional treatment models. IMT teams stay connected to individuals regardless of their location in shelter, on the street, in housing, hospital or jail.

Purpose of Position: The Psychiatric Nurse Practitioner (PNP) provides psychiatric evaluation, diagnosis, medication management, and treatment for IMT clients as part of a multidisciplinary team. The PNP supports recovery, safety, community stability, and improved health outcomes using recovery-oriented, trauma-informed, and harm reduction approaches.

Schedule: Flexible with fixed schedule, 17.5 Hours per week

Hourly Range: $115.00-$135.00/hr

Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidates number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

Qualifications

Roles, Responsibilities and Essential Duties

- Provide psychiatric evaluation, diagnosis, treatment planning, medication management, and reassessment for IMT clients.

- Prescribe, administer, monitor, and adjust psychiatric medications and medications for substance use disorders, including medication assisted treatment for substance use disorders (MAT) and long-acting injectable antipsychotics when clinically appropriate.

- Assess psychiatric, medical, substance use, medication adherence, side effects, and treatment response.

- Provide services in office, community, shelter, street, subway, hospital, supportive housing, carceral, and other field-based settings.

- Coordinate care with medical and substance use providers, hospitals, pharmacies, AOT, families, and other collateral supports as necessary.

- Provide psychiatric consultation, crisis support, clinical guidance, and urgent intervention support to clients and the multidisciplinary IMT team.

- Participate in team meetings, case conferences, supervision, clinical reviews, trainings, and shared case management activities.

- Maintain timely and accurate documentation in accordance with program, regulatory, and funder requirements.

- Other duties as required or assigned.

Education and Licensure Requirements:

- Current NYS Nurse Practitioner in Psychiatry license and active NYS registration required (PNP/PMHNP/PMHNP-BC).

- Collaborative practice agreement completion (>3600 hours).

- DEA registration or ability to obtain within 3 months of hire with prescriptive authority (including Buprenorphine).

Preferred Qualifications

- Experience in ACT, IMT, homeless outreach, mobile treatment, crisis services, harm reduction, Medication Assisted Treatment (MAT), or criminal justice/shelter/hospital systems preferred.

- Fluency in Spanish or American Sign Language (ASL) a plus.

Knowledge, Skill and Abilities

- Strong communication, psychiatric assessment, psychopharmacology, medication management, crisis assessment, clinical consultation, documentation, and care coordination skills.

- Ability to learn Microsoft 365, AWARDS, HEALTHIX, PSYCKES, EPIC CommunityLink, and other required electronic systems.

- Experience providing psychiatric evaluation, diagnosis, medication management, and services to individuals with serious mental illness, co-occurring substance use disorders, homelessness, and trauma required.

- Obtain and maintain MHL 9.58 designation.

Physical Requirements/Working Conditions

- Field work required throughout the five boroughs in varied community settings, weather, and environmental conditions.

- Walking, standing, sitting, typing, climbing stairs, and lifting up to 35 pounds required.

- After-hours/weekend phone availability to assist with urgent clinical matters.

- Fieldwork required; travel by MTA subway/bus required; valid insurable driver's license a plus
