Unregulated Care Provider (UCP):
CareKW is looking for experienced, skilled and reliable individuals to join our team as a UCP to provide support in Facility and Community settings.

CareKW is focused on creating a team of qualified individuals to enhance and advance the health of the community. We are open to UCP's with a variety of backgrounds and training expertise.

Shift Details:
Employment Type: Full time
Pay Range: $20 - $23/ hour
Shift timing: 8 hour - 12 hour rotational shifts between 6AM and 11PM
Schedule:10 shifts biweekly, every other weekend required

Responsibilities :

- Complete regular caregiver skill evaluations, address concerns as appropriate in collaboration with nursing team.

- Address client or caregiver queries or concerns related to the provision of care (in person or by telephone)

- Perform client intake assessments prior to the start of services and complete client update assessments as needed throughout the duration of service; communicate focus of intervention to client and family

- Create and maintain client care plans based on client’s identified care needs; communicate changes in client’s health status or discrepancies in assigned care tasks to community care coordinator

- Be flexible in covering open shifts and display willingness to adjust work schedule to ensure coverage in the community

- Work with clients to establish meaningful goals to increase their function at home

- Work as a dynamic part of a multidisciplinary team

- Assist with policy creation as needed

- Be responsive to emails and calls as needed

- Provide client-centered, family-centered care

- Must be available for rotating shifts.

Requirements:

- Previous experience in providing person care

- Preferred post-secondary education in related care fields of PSW/HCA/ DSW certification

- Hold a valid G drivers license and willing to commute between Brantford, St Catharines and Hamilton

- Willing to commute to Kitchener once per week

- Valid work/study permit (if applicable)

- Current CPR/First Aid Certification

- Clean Vulnerable Sector Screening (within 3 months of hire)

- Up-to-date Immunization Records (MMR, TDAP, Varicella, Hep B)

- Gentle Persuasive Approach (GPA) considered an asset

- Non-Violence Crisis Intervention (NVCI) considered an asset

- Documentation skills

- Ability to follow client care plan

- Strong communication skills

- Strong problem-solving skills

- Excellent patient service skills
