Nurse Case Manager

Job Title: Nurse Case Manager Job Description The Nurse Case Manager supports care coordination, administrative nursing functions, and operational workflows in a provider or payer environment. This non-clinical, non-patient-facing role focuses on facilitating communication between care teams, supporting population health programs, and contributing to process and quality improvement initiatives that align with organizational goals. Working within a team-based setting, the Nurse Case Manager enhances internal care processes through coordination, documentation, and collaboration, without performing direct patient care, invasive procedures, or exercising clinical judgment. Responsibilities

- Coordinate care management activities through administrative support and structured workflows.
- Collaborate with providers, care teams, and external partners to support continuity of care processes.
- Review and manage documentation, referrals, and care plans based on established guidelines.
- Provide general health education and resource information using approved materials.
- Route patient and provider inquiries to appropriate clinical or operational teams.
- Support workflow efficiency, scheduling alignment, and internal operational processes.
- Serve as a resource to internal teams on systems, coordination workflows, and administrative procedures.
- Contribute to quality improvement initiatives and assist with related reporting.
- Assist with onboarding coordination, scheduling, and team support functions.
- Participate in internal meetings and organizational initiatives to enhance care coordination and operations.
- Perform additional administrative duties as assigned to support healthcare operations.
- Maintain compliance with all organizational policies, procedures, and regulatory requirements.
- Maintain regular attendance and punctuality in accordance with organizational standards.

Essential Skills

- Active, unencumbered Registered Nurse (RN) license, with Kansas licensure preferred.
- Graduate of an accredited RN program.
- Strong communication skills to effectively interact with providers, care teams, internal staff, and external partners.
- Proven coordination and organizational abilities to manage documentation, referrals, and care plans.
- Ability to work in a structured, fast-paced environment with clear workflows and performance expectations.
- Strong time management and problem-solving skills to prioritize tasks and resolve operational issues.
- Cultural awareness and the ability to support diverse populations in a respectful and inclusive manner.
- Understanding of payer and provider operations or population health concepts (preferred).
- 2+ years of experience in care coordination, case management, utilization review, or healthcare operations preferred.
- Experience in payer, outpatient, or administrative healthcare environments preferred.
- Comfort with heavy use of computers, phones, and healthcare systems such as EMR/EHR platforms.

Work Environment This is an on-site, office-based role within a provider or payer organization, scheduled Monday through Friday, 8:00 AM to 5:00 PM. The environment is fast-paced and team-oriented, with a strong focus on coordination and administrative support rather than direct patient care. The role involves heavy use of computers, phones, and healthcare information systems such as EMR/EHR platforms, and regular collaboration with internal teams, providers, and external partners. There is no direct patient care, no clinical procedures, and no exposure to clinical treatment environments. Pay and Benefits
The pay range for this position is $25.00 - $35.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
