PRIMARY RESPONSIBILITIES:
Transplant Coordinator will coordinate the care of kidney and/or pancreas transplant recipients at the time of transplant and ongoing. Coordinator will work with transplant team and referring physicians to monitor progress and ongoing treatment of transplant recipients. Post-Transplant Coordinator is responsible for
post-operative teaching as well as ongoing education of transplant recipients. Assist in the operation of post transplant clinic hours.

ESSENTIAL FUNCTIONS:
•Coordinate ongoing care of kidney and/or pancreas transplant recipients following transplantation
•Monitor the progress and ongoing treatment of post-transplant recipients
•Provide post-operative teaching and continuous education to post transplant recipients
•Assist in the operation of post-transplant clinic
•Interacts with co-workers, visitors, and other staff consistent with the core values of the Hospital.

Job Description

EDUCATIONAL/TRAINING REQUIREMENTS:

Bachelor’s degree preferred

CERTIFICATES, LICENSES, AND REGISTRATION:

Registered Nurse required.  American Board Transplant Coordinator certification preferred

EXPERIENCE REQUIREMENTS:

Minimum of three years experience with renal and/or transplant patients

Strong communication and writing skills

Proficiency with Microsoft Office computer applications
