Job Title: RN Case Manager Job Description The RN Case Manager coordinates patient care across the continuum in an acute, Level I trauma hospital setting. This role focuses on conducting comprehensive initial assessments, developing safe and viable discharge plans, and collaborating closely with a multidisciplinary team to ensure appropriate, efficient, and patient-centered care. The RN Case Manager applies strong clinical judgment, case management methodologies, and utilization management principles to support high-quality outcomes and appropriate use of healthcare resources. Responsibilities
- Conduct thorough initial case management assessments for patients in observation or inpatient levels of care to identify discharge planning needs and care coordination requirements.
- Develop safe, viable, and individualized discharge plans in collaboration with patients, family members, and caregivers to ensure continuity of care after hospitalization.
- Coordinate care across the continuum by working closely with the multidisciplinary team, including physicians, nurses, therapists, social workers, and other care team members.
- Participate actively in multidisciplinary rounds to review patient progress, update plans of care, and refine discharge planning based on evolving clinical needs.
- Collaborate with utilization review nurses to monitor and support authorization of care, ensuring appropriate level of care and timely approvals.
- Partner with case management assistants to manage and facilitate the post-acute referral process, including placement in skilled nursing, rehabilitation, home health, or other post-acute services.
- Complete utilization reviews when needed or assigned, applying established criteria to support medical necessity and appropriate resource utilization.
- Ensure medical appropriateness and medical necessity criteria are met and clearly documented for level of care from admission through discharge.
- Apply a solid working knowledge of case management and utilization management methodologies and practices to daily decision-making and patient care activities.
- Use clinical expertise, discretion, and independent judgment in performing the nursing processassessment, care planning, intervention, and evaluationwithin a patient-centered care professional practice model.
- Develop and oversee implementation of an interdisciplinary plan of care that integrates an individualized, comprehensive discharge plan aligned with the patients ongoing healthcare needs.
- Reevaluate plans of care regularly and adjust discharge planning and care coordination strategies based on changes in the patients condition, needs, and goals.
- Engage in opportunities to directly or indirectly influence decision-making related to bedside clinical practice and case management processes.
- Maintain a positive mindset and adaptability in response to changes in workforce, clinical practice, and operational settings.
- Work efficiently and calmly in a fast-paced environment, responding courteously and professionally under pressure.
- Demonstrate strong communication and interpersonal skills when meeting with patients, families, and other designated participants in care to explain plans, address concerns, and support informed decision-making.
- Assist with authorization of post-acute care services, ensuring timely and appropriate transitions to the next level of care.
- Support a culture of continual learning and professional development within the case management team and broader care environment.
Essential Skills
- Active and unencumbered California Registered Nurse (RN) license.
- Bachelors degree in Nursing (BSN) required.
- Current Basic Life Support (BLS) certification provided by the American Heart Association.
- Minimum of 2 years case management experience in a Level I trauma facility.
- Minimum of 2 years inpatient acute care experience in a non-ED, non-clinic setting.
- Minimum of 1 year hospital case management or hospital utilization management experience.
- Demonstrated experience in discharge planning and care coordination in an acute care environment.
- Strong working knowledge of case management and utilization management practices, methodologies, and criteria for medical necessity and level of care.
- Knowledge of state and federal healthcare regulations and accreditation standards related to case management and utilization management.
- Ability to perform comprehensive assessments, develop care plans, implement interventions, and evaluate outcomes using the nursing process.
- Proven ability to work efficiently in a fast-paced, acute care setting while exercising independent clinical judgment.
- Excellent communication and collaboration skills for working with multidisciplinary teams, patients, and families.
- Ability to remain calm, courteous, and professional under pressure and during complex or challenging situations.
- Strong organizational and time-management skills to manage multiple patients, tasks, and priorities.
Additional Skills & Qualifications
- Nationally recognized certification in Utilization Management or Case Management is preferred.
- Experience in a large tertiary academic medical center is highly valued.
- Familiarity with professional practice models such as Duffys Quality-Caring Model or other patient-centered care frameworks.
- Experience with care coordination, utilization review, and post-acute referral processes in a trauma Level I acute care setting.
- Demonstrated ability to adapt to changes in clinical practice, workforce dynamics, and operational processes.
- Commitment to continual learning, professional growth, and participation in a large, collaborative case management team.

Work Environment This position is based in a large, 400-bed tertiary academic medical center with a Level I trauma designation and an acute care setting. The RN Case Manager works primarily on inpatient nursing units and participates in multidisciplinary rounds and team conferences. The schedule includes an AM shift, typically from 8:30 a.m. to 5:00 p.m., with a requirement to work every other weekend. The organization maintains a fast-paced, high-acuity environment where case managers collaborate closely with utilization review nurses, case management assistants, and a broad range of clinical disciplines. Pay and Benefits
The pay range for this position is $50.00 - $75.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
