Health Navigator (Remote)
Location: Remote (Must reside within 2 hours of Chattanooga, TN) Schedule: Monday-Friday, 8:30 AM-5:00 PM (30-minute lunch)
Position Overview
Our client is seeking a Health Navigator to support their clinical team and serve as a key resource for members regarding healthcare benefits, disease prevention, and wellness programs. This role focuses on helping members understand and navigate the healthcare system while ensuring they receive the support and resources available to them.
Key Responsibilities
- Serve as a liaison between members, healthcare providers, and insurance benefits.
- Conduct outbound calls to educate members on available benefits, services, and wellness programs.
- Complete health needs assessments and connect members with appropriate population health management programs.
- Reach out to members with identified gaps in care and encourage compliance with recommended healthcare services.
- Assist members with locating providers, scheduling appointments, and accessing healthcare resources.
- Manage work queues and review member eligibility, claims history, authorizations, and prior case activity.
- Coordinate member outreach and assign cases to the appropriate clinical team members.
- Research and resolve member inquiries, complaints, incentive issues, and other program-related concerns.
- Maintain accurate documentation and provide exceptional customer service.
Qualifications
Required
- Associate degree in Education, Communications, Healthcare, or a related field, or equivalent work experience.
- Minimum of 2 years of customer service experience.
- Strong verbal and written communication skills.
- Excellent organizational and time management abilities.
- Ability to work independently and manage multiple priorities.
- Strong problem-solving and decision-making skills.
- Knowledge of medical terminology.
Preferred
- CNA or CMA certification.
- Experience working with: Case managers
- Social workers
- Dietitians
- Behavioral health professionals
- Pharmacy or DME (Durable Medical Equipment) services
- Experience in case management, social work, behavioral health, or healthcare operations.

Job Type & Location
This is a Contract position based out of Chattanooga, TN.
Pay and Benefits
The pay range for this position is $22.00 - $22.00/hr. Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors. Eligibility requirements apply to some benefits and may depend on your job
classification and length of employment. Benefits are subject to change and may be
