How you’ll make an impact in this role

- Coordinates comprehensive inpatient rehabilitation plans, leading interdisciplinary team conferences with physicians, therapists, and social workers to track patient functional progress.
- Conducts ongoing utilization reviews and insurance authorizations to ensure appropriate level-of-care criteria, length of stay, and reimbursement compliance.
- Assesses complex post-discharge needs to arrange durable medical equipment (DME), home health, outpatient therapies, or alternative placement transitions.
- Serves as the primary liaison for patients and families, facilitating goals-of-care discussions, barrier resolution, and comprehensive discharge planning education

What minimum requirements you’ll need

Licensure / Certification / Registration:

- Licensed Clinical Registered Nurse credentialed from the Texas Board of Nursing required.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
- Inpatient rehab experience strongly preferred
