Part-Time LPN Pre-Admission Coordinator

Fully Remote | Massachusetts License Required |

Friday–Sunday | $38–$42/hour

You know how much work happens between receiving a home health referral and welcoming a patient into care. In this role, you’ll help make that transition smoother by reviewing referral records, tracking pending admissions, and following up on the information our clinical team needs to get started.

Elara Caring is seeking a part-time LPN Pre-Admission Coordinator with skilled home health experience.

Schedule (12 hours/week) and Pay

- Friday: 6–8 p.m.
- Saturday and Sunday: Approximately 5 hours each day
- Approximately 10–15 hours per week
- Fully remote
- $38–$42 per hour

What you’ll do

- Review incoming referrals for complete, accurate clinical documentation.
- Track patients awaiting discharge or surgery and follow up on outstanding items.
- Confirm physician information, orders, face-to-face documentation, and other details needed before admission.
- Work with intake, sales, referring facilities, and clinical teams to resolve missing information.
- Follow up on discharge orders and documentation needed to move referrals forward.
- Complete patient welcome calls or confirm they have been completed.
- Document updates and keep referral workflows moving in Homecare Homebase.
- Help ensure referrals are ready for scheduling and a timely start of care.

What you’ll ring

- Current Massachusetts LPN license
- Skilled home health experience and a working knowledge of the intake and referral process
- At least one year of practical nursing experience
- Strong attention to detail and the ability to manage several pending referrals at once
- Comfort learning and using electronic clinical systems; Homecare Homebase experience is a plus
