Crisis Intervention Specialist- 1st Shift

The following duties are normal for this position. These are not to be construed as exclusive or all‑inclusive. Other duties may be required and assigned.

- Provide ongoing assessment and treatment planning, case management, symptom management, medication monitoring, and court monitoring of clients with serious and persistent mental illness and co-occurring substance use.
- 70% of case management is performed in the community and in clients' homes
- Support clients' recovery through activities of daily living to include coaching and assistance in the areas of housing, money management, vocational/educational pursuits, and scheduling/transportation to appointments
- Recognizes signs of medication side effects or decompensation and reports immediately to the attending psychiatrist, encouraging the patient to immediately seek medical assistance. Encourages medication compliance.
- Participate in group facilitation (as determined by supervisor)
- Responsible for care coordination with clients' natural supports
- Working with teammates to complete all assigned and non-assigned tasks as part of client treatment
- Assessment, referral, and assistance with client healthcare and other treatment needs
- Knowledge of evidence based practices such as motivational interviewing and trauma informed care
- Maintain accurate and timely documentation in the form of assessments, treatment plans, progress notes, etc. as required by agency policy including the authorizing and distribution of funds when warranted
- Frequent home visits and transporting of clients in the community when other options are not available using county owned vehicle.
- Attend and participate in staffings and in-services
- Provides direct face-to-face (via walk-in clinic or mobile response) and telephone contact crisis intervention services. Services include evaluation, crisis counseling, information and referral to a wide variety of collateral agencies, families and resources. Social Worker responsible for arranging admission to an inpatient psychiatric facility for individuals subject to Chapter 51 detentions.
- Responsible for ensuring services provided by the least restrictive and clinically appropriate means and ensures treatment/recovery is in accordance with best practice and professional ethics and boundaries, and code conduct.
- Provide and arrange for provision of support services and collaboration with other community resources and providers. Evaluate the need for protective services, guardianships and alternative placements.

Desirable Knowledge and Abilities:

Must be willing to attend appropriate in-service education. Able to converse in a direct, supportive manner. Able to function under stress, to seek assistance when needed and to make decisions. Able to work cooperatively with diverse agencies, families and staff members. Able to preserve patient confidentiality. Must have current valid Wisconsin driver’s license.

Training and Experience:

A Bachelor's or Master's Degree from an accredited college or university in social work, psychology, human services or related field. Must have the ability to interact professionally with clients who may be challenging and potentially hostile. Experience working with children and families in the capacity of child welfare, child protective services, or juvenile justice a plus. A minimum of one year of experience working with youth in the area of mental health preferred. A valid Wisconsin driver’s license, available automobile, and automobile insurance are required.
