What You'll Do

- Communicate effectively with patients in both English and Spanish, including conducting assessments, providing case management support, explaining services and resources, and coordinating care.

- Provide culturally responsive support to Spanish-speaking patients and help address language and cultural barriers that may affect access to healthcare, housing, benefits, and community resources.

- Work collaboratively with members of the Outreach Team and primary care team to identify patients who need case management services.

- Conduct needs assessments for patients identified as medically vulnerable.

- Develop and regularly update patient-centered care plans that address areas of need identified through case management assessments.

- Coordinate the delivery of medical, behavioral health, housing, and supportive services for patients within the clinic and with community partners.

- Assist patients with linkages to community organizations and service providers who can help meet patient needs.

- Serve as the primary point of contact for patients transitioning to and from institutional settings to ensure continuity of care and coordinated reentry.

- Advocate for patients and assist them in navigating complex healthcare, social service, housing, and public benefits systems.

- Complete and maintain Utah State Case Manager Certification requirements.

- Maintain knowledge of clinic policies, procedures, systems, and electronic health records.

- Maintain case management data to facilitate reporting and quality improvement efforts.

- Maintain accurate, up-to-date records in the clinic's EMR system and ensure documentation supports billing requirements.

- Support and encourage a collaborative culture that values mutual respect, partnership, learning, and trust.

- Model positive and appropriate professional behavior.

- Demonstrate initiative and foster initiative in others.

- Participate in professional workgroups and organizations as needed.

- Perform other duties as assigned.

Required Qualifications

- High school diploma or equivalent.

- Social service, community outreach, case management, or related experience.

- Fluency in English and Spanish (verbal and written) required.

- Ability to work effectively with individuals experiencing homelessness, complex medical conditions, behavioral health challenges, substance use disorders, and other barriers to care.

Preferred Qualifications

- Medical case management or care coordination experience

- Experience working with individuals experiencing homelessness or in shelter, resource center, healthcare, or community-based settings.

- Knowledge of local housing, healthcare, and social service resources.

- Experience using trauma-informed, patient-centered, and culturally responsive approaches to service delivery.

Work Environment

- Standard hours: Monday-Friday, 8 am-6 pm

- Outreach requires travel to community sites or Homeless Resource Centers

- Physical requirements include extended periods of standing/walking and occasional lifting
