Service Coordinator - Assertive Community Treatment Team
Annual Salary: $52,278
Schedule: Monday - Friday, 8:30 am to 5:00 pm, with rotating weekends

Hampton-Newport News Community Services Board (H-NNCSB) is seeking a Service Coordinator to join our Assertive Community Treatment (ACT) Team. This is an opportunity to work alongside a multidisciplinary team supporting adults with severe and persistent mental illness through intensive, community-based services designed to promote stability, recovery, independence, and quality of life.
ACT brings together medical, behavioral health, and rehabilitative professionals to provide coordinated support to individuals who may need intensive services to remain successfully in the community.
Required qualifications:
- Bachelor's degree in Human Services such as Social Work, Psychology, Counseling.
- One (1) year of case management/service coordination experience with severe and persistent mental illness.
- QMHP registration (or QMHP-trainee registration) within 30 days of hire.
What You'll Do
As a Service Coordinator on the ACT Team, you'll help individuals access the services and resources they need while working closely with families, providers, and other professionals.
- Assess and coordinate services by completing intakes, eligibility determinations, mental status and social assessments, service plans, and ongoing reviews.
- Coordinate person-centered care by making referrals, arranging appointments and services, monitoring progress, supporting medication compliance, and participating in team case reviews.
- Advocate for individuals and families by connecting clients with housing, food, healthcare, disability benefits, transportation, and other community resources.
- Provide direct support and crisis intervention, including supportive counseling, emergency crisis response, problem-solving, and assistance with achieving individualized goals.
- Support continuity of care through discharge planning, housing resource development, collaboration with agency professionals, accurate documentation, and participation in court hearings or other legal proceedings as needed.
