Responsibilities

- Evaluate patients’ physical, cognitive, and functional abilities to identify areas that may affect their independence and quality of life

- Develop individualized treatment plans based on each patient’s needs, goals, and abilities

- Provide therapeutic interventions to improve patients’ ability to perform activities of daily living

- Help patients develop and maintain skills needed for bathing, dressing, grooming, meal preparation, and other daily activities

- Recommend and teach the use of adaptive equipment and techniques to promote independence and safety

- Assess patients’ homes and recommend modifications to reduce fall risks and improve safety

- Provide exercises and therapeutic activities to improve strength, coordination, balance, mobility, and functional independence

- Educate patients and caregivers on treatment plans, safety techniques, and strategies for continued progress at home

- Monitor patient progress and modify treatment plans as needed

- Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals

- Maintain accurate and timely documentation of patient evaluations, treatment plans, progress, and services provided

- Promote patients’ independence, dignity, and quality of life through compassionate, patient-centered care

- Follow all applicable professional, organizational, and regulatory standards

Requirements

- Proven work experience as a Physical Therapist

- Intensive education and clinical expertise

- Ability to manage patients with different types of personalities

- Current knowledge of treatment practices

- Drive to continually learn and grow

- Professional behavior

- Excellent interpersonal communication skills

- Valid license to practice physical therapy

- Graduate degree or greater in physical therapy
