The Primary Support Professional (Residential) works closely with the Program Coordinator to ensure the successful implementation of each individual's Individual Support Plan (ISP). This role coordinates day-to-day residential activities, supports client health and wellness needs, collaborates with team members, assists with medical appointments, promotes safety and quality standards, and helps ensure individuals receive the highest level of person-centered support.

Key Responsibilities:
· Coordinate and support the implementation of each individual's Individual Support Plan (ISP) to help them achieve personal goals and maintain quality of life.
· Foster positive relationships with clients while providing person-centered support in residential and community settings.
· Transport and accompany individuals to required medical appointments and assist with follow-up care recommendations.
· Train and support staff regarding client medical needs, personal preferences, medications, adaptive equipment, and special dietary requirements.
· Review and monitor residential documentation, daily notes, goal tracking, health records, medication administration records (MARs), and related documentation for accuracy and compliance.
· Ensure residential homes maintain health, safety, cleanliness, and emergency preparedness standards, including monitoring safety kits, food storage, and household conditions.
· Coordinate staffing coverage and scheduling needs to maintain continuity of care and support services.
· Collaborate with Program Coordinators, nursing staff, guardians, and quality management personnel to address follow-up actions and client support needs.
· Monitor client finances by ensuring receipts, ledgers, and money counts are accurately maintained according to established procedures.
· Support positive behavioral interventions and participate in ISP meetings, RN visits, service monitoring visits, and other care coordination activities.
· Ensure safety drills, incident reporting, and required monthly documentation are completed in accordance with organizational and state requirements.
· Perform other duties as assigned.
Education, Experience, and/or Credential Qualifications:
· High school diploma or GED required.
· One (1) year of experience working with individuals with intellectual and developmental disabilities preferred.
· In lieu of experience, successful completion of Missouri Quality Outcomes training approved by the Division of Developmental Disabilities regional office may be accepted.
Additional Qualifications:
· Must be at least 18 years of age.
· Current valid driver's license required.
· Acceptable driving record required.
· Current automobile insurance required.
· Successful completion of background screenings, including criminal record, driving record, abuse/neglect, and fingerprint checks.
· Ability to read and interpret safety rules, operating instructions, procedures, and related documentation.
· Ability to prepare routine reports and correspondence.
· Ability to communicate effectively with individuals served, family members, coworkers, and community partners while utilizing person-centered language.
· Ability to follow written, verbal, and diagram-based instructions.
· Strong problem-solving and organizational skills.
· Reliable means of communication required.
· Ability to read, understand, and prepare clear written documentation.
Physical Requirements
· Perform sedentary work, occasionally exerting up to 10 pounds of force to lift, carry, push, pull, or move objects.
· Utilize repetitive hand, finger, and arm movements for typing, writing, and documentation throughout the workday.
· Sit for extended periods of time while performing administrative and documentation duties.
· Occasionally stand and walk for brief periods as required to support daily job responsibilities.
