Job Description : The PRN Licensed Practical Nurse will evaluate our clients for emergent care needs and support our therapeutic team within patient treatment plans. The PRN LPN will document the medical progress and notes while keeping up with all state and federal regulation. Those hired in this role will be required to work at least four (4) shifts per month to maintain Active employment status. Selected candidates could be assigned to work at any WellStone location as needed.
Duties and Responsibilities:
- Conduct vital signs, i.e. orthostatic blood pressure, weight, height, pulse rate information on involuntarily committed patients of our 16-bed locked down facility or patients at our 10-patient facility
Make inquiries of client to include his/her physical status, any changes with him/her, symptoms, etc.
- Administer, monitor, and assess stat I.M. or P.O. medication, then document accordingly in electronic medical records in a timely manner.
- Assess medication side effects and/or adverse reactions.
- Perform crisis intervention techniques as needed.
- Interface with local laboratories to obtain test results.
- Comply with OSHA regulations when administering I.M. medications.
- Complete and maintain documentation in a timely manner.
- Ensure required training is up to date.
- Provide oversight to the Behavioral Aide working with the clients providing Basic Living Skills.
- Attend staff meetings
- Prepare discharge planning
- Maintain licensure in the State of Alabama
- Complete other duties assigned.

- Valid AL Nursing License
- Minimum of two years of mental health experience
- Excellent Verbal and Written Communication Skills
- Excellent organizational & planning skills
