Overview

Correctional Healthcare Partners (CHP) is seeking a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver high-quality, patient-centered care within a correctional setting.

This role offers the opportunity to practice at the top of your license, providing comprehensive medical care to an underserved population while working collaboratively within a multidisciplinary team. The APP will play a key role in improving patient outcomes, ensuring continuity of care, and supporting CHP’s mission of delivering accessible, quality healthcare.

Key Responsibilities

- Perform comprehensive health assessments, including physical exams, medical histories, and diagnostic evaluations

- Diagnose and manage acute and chronic medical conditions

- Develop and implement individualized treatment plans

- Prescribe medications and treatments in accordance with state regulations and CHP protocols

- Order, interpret, and evaluate diagnostic tests and lab results

- Provide patient education on disease prevention, health maintenance, and treatment plans

- Respond to medical emergencies and provide urgent care as needed

- Collaborate with physicians, nursing staff, mental health clinicians, and custody staff to ensure coordinated care

- Maintain accurate, timely, and compliant documentation in the electronic health record (EHR)

- Participate in quality improvement initiatives and adherence to clinical protocols

- Support discharge planning and continuity of care upon release or transfer

Qualifications

- For Nurse Practitioners (NP):

Active and unrestricted NP license in the applicable state

- Board certification (FNP, AGNP, or related)

- Current DEA license (or ability to obtain prior to start)

- For Physician Assistants (PA):

Active and unrestricted PA license in the applicable state

- Graduate of an accredited PA program

- NCCPA certification required

- Current DEA license (or ability to obtain prior to start)

- Minimum 2–3 years of APP experience in primary care, urgent care, emergency medicine, or a related setting

- Experience in correctional healthcare or working with underserved populations preferred but not required

- Strong clinical assessment and decision-making skills

- Ability to work independently within scope while collaborating with supervising physicians and interdisciplinary teams

- Excellent communication and interpersonal skills

- Practice in a structured, secure correctional facility

Location: San Diego, CA

Pay: $95-$100/hr

Schedule: Day shift, 12 hour shifts, 6am-6pm-Weekday & Weekend shift variability (Sat/Sunday)
