About the Role
Matter Health is seeking a Nurse Practitioner to provide community-based primary care in Nashville, Tennessee .
At Matter Health, our NPs are more than just providers. They are Panel Owners . In this role, you will manage a dedicated panel of patients as their ongoing primary care provider, building strong, lasting relationships within the communities you serve.
This is a fully community-based position with no traditional clinic home base. Nurse Practitioners work with a high degree of clinical autonomy while collaborating closely with an interdisciplinary care team and a dedicated Medical Director for support in complex cases.
Key Responsibilities
- Provide ongoing primary and preventive care to adult patients living in affordable housing communities
- Manage and grow a dedicated patient panel within assigned communities
- Deliver relationship-driven, patient-centered care in community and in-home settings
- Collaborate with interdisciplinary clinical and operational teams
- Support initiatives in preventive care, chronic disease management, and care coordination
- Use a combination of telehealth and in-person care delivery to improve accessibility
- Maintain accurate, timely EHR documentation and ensure coding specificity
Qualifications
- Active, unrestricted Tennessee Nurse Practitioner license , or the ability to obtain one before your start date
- Tennessee Certificate of Fitness to prescribe , or the ability to obtain one before your start date
- National board certification as a Nurse Practitioner through AANPCB or ANCC in family, adult, geriatric, or adult-gerontology
- Active DEA registration , or the ability to obtain one before your start date
- 3+ years of clinical experience as a Nurse Practitioner, preferably with medically complex adult or geriatric populations
- Basic Life Support (BLS) certification
- Valid driver’s license and reliable transportation, with monthly travel stipend provided
- Experience with EHR documentation , preferably athenahealth
- Commitment to equitable, patient-centered care
Strongly Preferred
- Background in family medicine , internal medicine , adult medicine , or geriatrics
- Experience in value-based care settings
- Experience providing care in community-based , in-home , or other non-traditional healthcare environments
- Experience supporting patients with behavioral health needs alongside chronic conditions
- Familiarity with MEAT documentation and chronic condition coding
- Telehealth experience
