Job Summary:
This full-time RN position manages clinical resources for the Big Bend Hospice (BBH) interdisciplinary Group (IDG) and determines the daily utilization of staff and resources. Enhances clinical practice of the team by performing as a role model, mentor, instructor, and resource person.

Supervisory Responsibilities:
This position supervises assigned clinical staff (RN, LPN, CNA and CSS)
Duties/Responsibilities:
Primary clinical and administrative resource for the Interdisciplinary Group (IDG)*
o Determines the daily utilization of IDG staff
o Appropriately triage clinical calls to IDG members
o Provides coverage/assistance to members as needed and as patient acuity dictates to ensure continuity of care to all patients on the team
o Participates in the team responsibility of providing 24-hour/day, 7-day/week patient care as needed.
o Participates in Manager on-call staffing as needed.
o Promotes adherence to Big Bend Hospice (BBH) policies and procedures. Recommends alternate approaches to patient care as indicated
o Obtains reports and monitors ongoing assessments of physical, psychological, social, and spiritual needs of the patient, family, and patient caregiver and initiates appropriate intervention or referral in collaboration with IDG
o Provides education to IDG members regarding symptom control, case management, BBH, community, and financial resources
o Enhances the clinical practice of IDG staff by performing as a role model, mentor, instructor, and resource person
o Administers direct care to select patients, as deemed appropriate, to maintain own clinical competence and/or to access patient requirements and to provide modeling for staff
Assists in determining the appropriate financial classification for patients throughout their continuum of care, i.e., hospitalizations, respite, nursing home placement, revocations, etc.*
Responsible for daily operations of the Interdisciplinary Group (IDG)*
o Collaborates with 24-hour Care leadership team to plan on-call staffing schedule.

o Plans, organizes and coordinates IDG meetings.
o Prepares the IDG agenda.
o Collaborates with other departments, On-Call Nurse and Admissions, to ensure all patient/family care needs are met.
o Provide direct patient care as needed and assigned.
Utilizes appropriate communication methods and tools to relay direction and information to team members, co-workers, leaders, patients, families and vendors.*
o Communicates in a clear and accurate manner. Provides constructive feedback and resolves conflict directly. Assists others to improve group communication skills
o Assists in providing assessment, planning, implementation, and evaluation of team goals
o Incorporates joint decision-making and unified goal achievement into IDG practice
o Demonstrates respect and understanding for all clinical disciplines required for quality patient care
Responsible for management of assigned staff performance and development.*
o Recognizes, reinforces, and enhances the role of all disciplines in the IDG. Promotes the professional development of all members of the IDG.
o Assists in reviewing patient care documentation and providing on-site clinical supervisory visits to identify strengths and areas of development for the RNs/LPNs and Home Health Aides (HHA)
o Assists team members in collecting data related to monitors identified
o Provides data and findings for reporting to the CQI Committee
o Assists in the development of team-based education programs for the improvement of patient care
Represents Big Bend Hospice at community activities.*
Performs other duties as assigned by the Clinical Director.*

Education and Experience:
Current RN license, with a current Florida and/or Multistate compact license.
[BSN preferred]
Minimum of two [2] years of Hospice experience
Oncology or home health nursing experience preferred with at least one [1] year of acute and medical-surgical nursing
Must maintain current valid Florida licensure as an RN or compact licensure as an RN
Must possess a valid Florida Drivers' License, current auto insurance, and reliable transportation

Physical Requirements:
Standing: 66-100% of the shift
Sitting: 0-33% of the shift
Walking: 66-100% of the shift
Lifting: 0-50 lbs. as needed
Carrying: 0-25 lbs. as needed
Pushing: 0-250 lbs. on wheels with assistance as needed
Climbing: 1-5 flights of stairs as necessary
Pulling: 0-250 lbs. on wheels with assistance as needed
Bending: Proper bending as necessary to pick up things off the floor
Squatting: As necessary while providing patient care
Rotating: Shoulders as necessary
Crawling: If necessary
Kneeling: As necessary while providing patient care
Reaching: Overhead while providing patient care
Visual and hearing senses must be adequate to enter data in computers and mobile
devices
Utilize telephones and perform detailed paperwork.
