Registered Nurse
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Hours: 40 hours/week
Work Arrangement: 90% Remote / 10% Onsite
Job Summary
We are seeking an experienced Registered Nurse to support members through the Arizona Division of Developmental Disabilities (DDD). This field-based position will complete nursing assessments, document medical necessity, coordinate services, and collaborate with members, families, providers, and multidisciplinary teams.
Responsibilities
- Complete member reports and nursing assessments according to DDD requirements.
- Conduct in-person assessments every 90 days and as needed.
- Document medical necessity for skilled services.
- Complete nursing referrals and vendor calls.
- Participate in multidisciplinary planning meetings.
- Collaborate with members and their support systems.
- Educate members and families regarding complex medical information.
- Complete documentation within required timeframes.
- Attend mandatory meetings and complete annual training.
- Assist with special projects as needed.
Requirements
- Active Arizona RN license.
- 4–5 years of recent RN experience in home health, hospice, long-term care, or case management .
- Recent experience caring for individuals with feeding tubes, including gastrostomy (G-tubes) .
- At least 2 years of experience working with individuals with disabilities.
- Valid Arizona Driver’s License.
- Arizona Fingerprint Clearance Card required.
- Moderate to advanced experience with Microsoft Word, Google Suite, and Adobe .
- Strong communication, documentation, and collaboration skills.
- Behavioral health, complex medical, or nursing case management training/certification preferred.
