Salary: £47,571 - £50,072 per annum (Inclusive of a 7.5% Market Supplement)

Hours per week: 37 hours

Our Continuing health care team are looking for an advanced practitioner. The Continuing Healthcare (CHC) team works across the county to assess and review adults who may be eligible for NHS Continuing Healthcare funding. The team works closely with health and social care partners, providing specialist advice on eligibility, funding arrangements, and care planning to ensure individuals receive the appropriate support and funding.

The Advanced Social Work Practitioner will lead on complex Continuing Healthcare and joint funding casework , support and supervise staff, promote high-quality social work practice and ensures compliance with safeguarding and relevant legislation. They will work closely with health and social care partners, provide professional guidance, and help improve services and outcomes for vulnerable adults.

The successful candidate will be a qualified and registered Social Worker with extensive experience in adult social care, safeguarding, and Continuing Healthcare. Skilled in managing complex cases, supporting staff development and delivering high-quality, person-centred services through strong communication, leadership and professional practice.

This position comes with a 7.5% market supplement payment. All market supplement payments are subject to review on an annual basis.

Our hybrid working model means office staff are on site for at least half their hours, community-based staff carry out at least half their admin time in a workplace, and customer facing roles work entirely from their service location.
